腓肠神经营养血管蒂逆行岛状皮瓣的临床应用

目的:探讨应用腓肠神经营养血管蒂逆行岛状皮瓣修复小腿下端、踝周和足部皮肤软组织缺损的可靠性.方法:对18例小腿下端、踝周和足部皮肤软组织缺损病人选择腓肠神经营养血管蒂逆行岛状皮瓣修复,皮瓣最大17cm×11cm、最小6cm×5cm.结果:皮瓣Ⅰ期愈合15例,Ⅱ期愈合3例.随访6个月～2年,皮瓣质地、颜色良好.结论:腓肠神经营养血管蒂逆行岛状皮瓣血管蒂恒定,层次清晰,操作简单,皮瓣不牺牲主干血管,对小腿功能和外形影响小,皮瓣转移幅度大,是修复小腿中下段、踝周、足跟等软组织缺损的良好修复方式.

Clinical Application of Reversed Sural Nutrition Neurovascular Pedicle Cutaneous Island Flap

Objective： To explore the reliability of repairing tissue defect of lower leg, ankle and foot with reversed sural neurovascular pediele cutaneous island flap. Method： 18 patients of tissue defect at lower leg, ankle and foot were repaired with reversed sural neurovascular pedicle cutaneous island flap, and the size of skin flap was from 6cm ×5cm to 17cm × 11 cm. Result： 15 cases reached to Ⅰ stage healing of skin flap; 3 cases was Ⅱ stage healing. Texture and color of skin flaps appeared better after 0. 5 - 2 years of follow - up. Conclusion ： Vascular pedicle of reversed sural neurovascular cutaneous island flap is invariablenes, stratum layer clear and operated easy and simply. Skin flap has few influences for trunk blood vessel, leg＇s function and appearance. due to skin flap transferable in large range.
