CT诊断乳腺肿瘤临床价值的研究

目的：探讨乳腺肿瘤的CT表现及临床应用价值。资料与方法：45例乳腺肿瘤病人行仰卧位平扫及增强扫描，小病灶行单侧局部放大扫描，其中43例经病理证实，另2例经临床随访证实。结果：乳腺癌的主要CT特征为：（1）肿块边缘不规整，有分叶和毛刺；（2）强化后CT值平均升高45Hu以上，无包膜；（3）肿块内有沙砾状钙化；（4）腋窝淋巴结境界欠光整，可有融合，呈“串珠状”改变；（5）肿块侵犯胸壁等。乳腺良性肿瘤境界光整，可有分叶和包膜。结论：CT是鉴别乳腺良恶性肿块的一种重要方法，并能发现临床扪诊阴性的乳腺病变，为乳腺癌术前分期及手术方式选择提供重要依据。

Study on diagnosis and clinical applicable value of Computed tomography in breast tumors

Purpose:To evaluate the manifestations and clinical applicable value of CT examination in breast tumors. Materials and methods: 45 Patients with breast tumors were checked upby plain scan and contrast scan of CT in supine position. The minor lesions were scaned and magnified unilaterally and locally. of the 45 cases, 43 cases were proved by pathology and 2 cases byfollow-up. Results:The main feature of CT images in breast cancer are as follows: (1)a lobulation mass displayed with irregular margin and stellate contour; (2)on enhancement scan, the CTvalues increase 45 Hu or more on the average more than that of plain scan; (3)a calcific like sandmass displayed with no entire integument; (4)axillary lymph nodes displayed with irregular margin and fusion like a string of beads; (5)a mass infiltrating the wall of chest and so on. The benign masses with entire integument were displayed with smooth margin and they may be lobulated. Conclusion: CT examination is a important method in differential diagnosis of breast lumps.The lesions proved negative by palpating in the opposite breast may be detected. CT examinatonwill provide the important evidences that may help decide breast cancer staying and elect themodes of operation.
