门静脉周围积液的影像表现分型及临床意义

摘    要:目的探讨肝门静脉周围间隙积液的影像学表现、分型及临床意义。方法选取265例经MSCT及MRI检查诊断肝脏IPSE征象的病例包括肝肿瘤介入治疗后108例(40.8%)、慢性肝炎78例(29.4%)、肝转移瘤25例(9.4%)、尿毒症16例(6.0%)、肝脓肿11例(4.1%)、心力衰竭10例(3.7%),其他包括急性胰腺炎6例、外伤性肝损伤6例和胰腺及胆囊术后5例(6.4%),总结IPSE的影像表现并分型。结果依照IPSE的影像表现分为三型:全肝叶型138例(52.1%);肝右叶型77例(29.1%),肝左叶型50例(18.8%)。结论不同类型IPSE影像表现与其发病基础有关,对临床综合判断肝组织及其功能损伤程度、分析病因及指导治疗有重要意义。

Abstract:Objective To investigate the imaging features and clinical significance of perihepatic portal space effusion.Methods 265 cases with hepatic IPSE signs detected by MSCT and MRI were collected.The clinical diagnosis involved diseases including 108 cases(40.8%)of liver tumor after interventional therapy,78 cases(29.4%)of chronic hepatitis,25 cases(9.4%)of liver metastasis,16 cases(6.0%)of uremia,11 cases(4.1%6)of liver abscess and 10 cases(3.7%)of heart failure.There were 6 cases with acute pancreatitis and 6 cases with acute pancreatitis.Results According to the imaging features of IPSE,there were 138 cases(52.1%)of whole hepatic lobe type,77 cases(29.1%)of right lobe type and 50 cases(18.8%)of left lobe type.Conclusion The imaging findings of different types of IPSE are related to the pathogenesis,which is of great significance for clinical comprehensive judgment of liver tissue and function damage,analysis of etiology and guidance of treatment.
