MRI analysis of acute leukoencephalopathy caused by methotrexate

Objective To explore the MRI imaging findings of leukoencephalopathy in patients with osteosarcoma caused by intravenous injection of high-dose methotrexate.Methods We collected 4 patients with osteosarcoma who underwent high-dose intravenous methotrexate chemotherapy in our hospital with induced acute leukoencephalopathy.All patients underwent cranial MRI plain scan and diffusion weighted imaging examination when they developed clinical symptoms,2 patients underwent MRI enhancement examination.We combined the clinical symptoms of the patients and analyzed their imaging manifestations.Results Hyperintensity was observed in the Centrum semiovale and the lateral ventricle on the MRI diffusion weighted imaging sequence of 4 patients with leukoencephalopathy.T2FLAIR images of 1 patient showed slightly hyperintensity,and 2 patients underwent enhanced MRI,the lesion had no enhancement.Re-examining the cranial MRI after 1~2 weeks,the results showed that these high signal areas of diffusion-weighted imaging had shrunk and disappeared.Conclusion The cranial MRI examination has great medical value for the early recognition of methotrexate-induced leukoencephalopathy,which is helpful for accurate diagnosis and timely appropriate treatment,and the prognosis is good.
