CalliSpheres载药微球TACE联合微波消融治疗原发性大肝癌疗效评价

目的探讨CalliSpheres载药微球经导管肝动脉化疗栓塞术(TACE)联合微波消融治疗原发性大肝癌的临床疗效。方法23例直径5~10cm原发性肝癌患者施行了CalliSpheres载药微球TACE联合微波消融治疗。术后1、3、6、9、12个月及以后每3个月采用肝脏增强MRI随访。结果23例患者共行33次CalliSpheres载药微球TACE联合微波消融治疗,无肝脓肿、胆脂瘤等并发症发生。23例患者随访3~22(平均9.2±3.9)个月,术后1、3、6、9、12和15个月客观缓解率分别为86.9%、87%、93.8%、77.0%、63.6%、57.2%,疾病控制率分别为100.0%、95.7%、93.8%、92.3%、81.8%、85.7%。术后3、6、9、12和15个月生存率分别为100%、93.8%、84.6%、81.8%、71.4%。结论CalliSpheres载药微球TACE联合微波消融治疗原发性大肝癌安全可靠,近中期疗效较好。

Clinical evaluation of transcatheter arterial chemoembolization using callispheres beads combined with microwave coagulation for treatment of massive primary hepatocellular carcinoma

Objective To evaluate the efficacy of transcatheter arterial chemoembolization(TACE)using CalliSpheres beads combined with microwave coagulation for treatment of massive primary hepatocellular carcinoma.Methods Twenty-three patients with primary hepatocellular carcinoma(diameter 5~10 cm)underwent CalliSpheres drug-eluting beads TACE combined with microwave coagulation.Liver dynamic enhanced MRI was followed up at 1,3,6,9 and 12 months after treatment and every 3 months thereafter.Results A total of 33 times of CalliSpheres drug-eluting beads TACE combined with mircrowave coagulation was performed in 23 patients without complications such as liver abscess and cholesteatoma.The 23 patients were followed up for 3~22(mean 9.2±3.9)months.The objective response rates at 1,3,6,9,12 and 15 months after the combination treatment were 86.9%,87%,93.8%,77.0%,63.6%and 57.2%,respectively,and the disease control rates were 100.0%,95.7%,93.8%,92.3%,81.8%and 85.7%,respectively.Postoperative survival rates at 3,6,9,12 and 15 months were 100%(23/23),93.8%(15/16),84.6%(11/13),81.8%(9/11)and 71.4%(5/7),respectively.Conclusion CalliSpheres drug-eluting beads TACE combined with microwave coagulation for treatment of massive primary hepatocellular carcinoma is an effective method with high safety and reliable short and mid-term outcomes.
