高频超声在糖尿病患者颈部血管检查中的诊断价值

摘    要:目的:探讨高频超声在糖尿病患者颈部血管检查中的诊断价值。方法回顾2010年3月至2013年9月260例糖尿病患者的临床资料,运用高频超声对双侧颈动脉内鄄中膜及其斑块进行检测,并对斑块引起的血管狭窄与血流动力学进行测量。同期246名健康体检者作为对照组,比较2组的检测结果,进行统计学分析。结果糖尿病组颈动脉内鄄中膜厚度、斑块发生率明显高于对照组,2组患者有不同程度的动脉粥样硬化形成,斑块部位多见于颈动脉分叉处,其次为颈内动脉起始段,对糖尿病组中出现斑块形成的166例患者进行血流速度与管腔狭窄程度的分析,得出斑块较小者无明显血流速度改变;斑块较大者引起管腔狭窄,血流增快。结论高频超声能准确检测出糖尿病患者颈动脉内鄄中膜厚度,斑块存在部位及病变范围,并提供斑块的形态学信息,能定量分析斑块所致动脉狭窄的程度,为糖尿病颈部血管病变的早期诊断治疗及预后提供了重要依据。

Abstract:Objective To investigate the diagnostic value of high frequency ultrasound in diabetes neck vascu-lar examination. Methods Retrospectively analyzed the clinical data of 260 samples with diabetes mellitus, exam bi-lateral carotid artery intima and plaques by high frequency ultrasound, and measured the stenosis and blood flow in-duced by plaque, and then compared them with 246 samples of healthy subjects by statistical analysis. Results The incidence of carotid intima-media thickness, plaque of patients with diabetes mellitus was significantly higher than healthy bodies. Two teams of patients had different level of atherosclerosis. The plaque commonly existed in carotid artery bifurcation, secondly in the initial segment of internal carotid artery. Comparing to other position, this difference has statistical significance. Analyze the degree of blood flow velocity and lumen stenosis for 166 patients who have been diagnosed plaque, we come to the conclusion that the patients who have smaller plaque have no significant changes in blood flow velocity, while the patients who have larger plaque will lead to luminal stenosis and increased blood flow speed. Conclusion High frequency ultrasound can accurately detect carotid artery intima-media thick-ness, position and extent of the lesion plaque for patients with diabetes mellitus, which will supply important basis for early diagnosis, treatment and prognosis of cervical vascular disease in diabetes.
