摘    要:目的通过对急性脊髓损伤患者使用不同剂量的单唾液酸四己糖神经节苷酯钠(GM-1)的疗效进行比较,观察GM—l的剂量对急性脊髓损伤的疗效的影响。方法回顾性分析我院2006年12月~2012年12月82例急性脊髓损伤病例,按照使用的药物剂量分3个小组:0剂量组(未使用)、低剂量组(20—40mg/d)、高剂量组(100mg/d);再按照美国脊髓损伤学会(ASIA)92损伤分级分为A、B、C、D级。对伤后24h、2周、1个月、2个月时患者的浅感觉、运动及小便功能等方面进行对比来评价不同剂量的GM-1对急性脊髓损伤严重程度的疗效影响。结果3组患者的用药前后浅感觉和运动功能的恢复评分各进行q检验,浅感觉和运动功能的恢复疗效均存在着显著性的差异(P〈0.05),具有统计学意义。3组患者用药前后的小便功能分级结果进行X。检验,小便功能恢复疗效有显著性差异(P〈0.05),具有统计学意义。结论GM-1对脊髓损伤患者的神经功能的恢复有促进作用,同时具有浓度依赖效应。大剂量的GM.1对脊髓损伤后的神经功能的恢复促进作用更加明显,尤其是在损伤的急性期。

Abstract:Objective To observe the clinical curative effect of different doses of Monsialotetrahexosyl Gan glisoside( GM1 ) in the treatment of acute spinal cord injury. Methods Eightytwo acute spinal cord injury pa tients who were admitted into our hospital from Dec. 2006 to Dec. 2012 were retrospectively analyzed. The 82 pa tients were divided into A,B,C,D class based on injury scale with ASIA 92 and were divided into three groups rel ying on the drug dose used :0dose group,low dose group(2040mg/d) and large dose group( 100rag/d). The su perficial sensory,motor,urinary function of patients at 24 hours, 2 weeks, 1 month and 2 months after injury were compared so as to evaluate the effect of different doses of monosialotetrahexosyl ganglisoside in treating acute spinal cord injury. Results After treatment, the efficacy recovery of shallow sensory and motor function in the three groups was significant with qtest ( P 〈 0. 05 ) , which had statistical significance. The efficacy recovery of urine func tion in the three groups was significant with X2test ( P 〈 0. 05 ) , which had statistical significance. Conclusion GM1 can promote the recovery of neurological function and has concentrationdependent effect for patients with spi nal cord injury. Moreover,large doses of GM1 used in spinal cord injury patients can better promote the recovery of neurological function, especially in the acute phase of iniurv.
