早期强化降压对高血压脑出血患者血肿扩大的影响

目的探讨早期强化降压对高血压脑出血（ICH）患者血肿扩大的影响。方法回顾高血压ICH患者88例，根据患者入院时收缩压分三组，Ⅰ组：140mmHg≤SBP≤159mmHgⅡ组：160mmHg≤SBP≤179mmHg；Ⅲ组：SBP≥180mmHg。根据收缩压控制情况分两组：A组：SBP〈160mmHg；B组：SBP≥160mmHg。把血肿体积扩大140％或者12．5cm^3，判定为血肿扩大。结果28例发生血肿扩大。入院时收缩压≥180mmHg患者较易发生血肿扩大，血压控制下收缩压≥160mmHg的患者也易发生血肿扩大，收缩压〈160mmHg患者血肿较少扩大；入院后SBP≥160mmHg的患者比SBP〈160mmHg的患者血肿扩大的发生率要高（P〈0．05）。结论高血压ICH早期强化降压治疗对控制血肿扩大有益。

Effect of Early Antihypertensive Treatment on Hematoma Enlargement of Patients with Hypertensive Cerebral Hemorrhage

Objective To discuss the effect of early antihypertensive treatment on hematoma enlargement of patients with hypertensive cerebral hemorrhage. Methods A retrospective analysis was made to clinical data of 88 cases with hypertensive cerebral hemorrhage; according to the systolic blood pressure at admission, the patients were divided into 3 groups： Group Ⅰ （ 140mmHg ≤ SBP ≤ 159mmHg ）, Group Ⅱ（ 160mmHg ≤ SBP ≤ 179mmHg ） and Group Ⅲ （ SBP 1≥180mmHg ） while based on the controlled systolie blood pressure, the patients were divided into 2 groups： Group A （SBP 〈 160mmHg） and Group B （SBP≥160mmHg） ; hematoma enlargement over 140% or 12.5cm2 was decided as hematoma enlargement. Result 28 cases had hematoma enlargement; patients with systolic blood pressure≥ 180mmHg at admission and patients with controlled systolic blood pressure≥ 160mmHg were likely to having hematoma enlargement; the occurrence of hematoma enlargement in patients with SBP ≥160mmHg after admission was higher than that in patients with SBP 〈 160mmHg（ P 〈 0. 05 ） . Conclusions Early antihypertensive treatment is effective in controlling hematoma enlargement of the patients with hypertensive cerebral hemorrhage.
