肱骨近端骨折两种方法治疗后冻结肩及干预措施

目的探讨肱骨近端骨折两种方法治疗后冻结肩情况以及干预方法。方法我院于2007年12月～2010年2月共收治64例肱骨近端骨折的患者,其中男30例,女34例,年龄23～96岁,平均54.1岁,肱骨近端锁定钢板手术治疗32例,保守治疗32例,按美国肩肘外科评分随访观察各病例肩关节恢复情况。结果 64例中30例出现不同程度冻结肩症状,肱骨近端骨折保守治疗与手术治疗以及同一治疗组内不同骨折类型之间评分有明显的差异（P〈0.05）。结论肱骨近端骨折出现冻结肩症状发生率高,治疗非常困难,需引起重视。

Study of frozen shoulder after proximal humerus fracture dealing with two methods

Objective To evaluate the rate of the frozen shoulder after treatment of proximal humerus fracture with two methods and treatment of this frozen shoulder.Methods From Dec 2007 to Feb 2010,64 cases who were proximal humeral fracture including 30 men and 34 wowen were treated in the Second Affiliated Hospital of Soochow University.The average age was 54.1 years old（from 23 to 96 years old）,16 cases were treated with operation which used the locking proximal humerus plate,16 cases were treated with conservative treatment.American Shoulder and Elbow Surgeons Scale was observed to evaluate the function recover.Results There were 30 cases in the 64 patients which were frozen shoulder.The difference of score between operation and conservative treatment in the proximal humerus fracture was significant（P0.05）.The score in different kinds of proximal humerus fracture treated with the same method was significant（P0.05）.Conclusion The rate of frozen shoulder after proximal humerus fracture and treatment is high.It is difficult to deal with.
