Study on the Validity of Detecting Stapes Malformation with Multi-Slice Spiral CT

Abstract:Objective To evaluate the ability of multi-slice spiral computed tomography(MSCT) in the detection of stapes malformation(SM). Methods The MSCT images of 32 patients with simple stapes malformation confirmed by surgery from November 2014 to December 2020 were retrospectively analyzed. The surgical findings were taken as the gold standard, and the ability of showing stapes deformity was evaluated based on standardized reconstruction images in transverse and coronal positions and axial images of stapes. Results A total of 32 sides of SM were included, among which 65.63%(21 sides) were abnormal stapes anterior crus, 59.38%(19 sides) were abnormal stapes posterior crus, 50.00%(16 sides) were abnormal stapes head, 43.75%(14 sides) were fixed stapes floor. The detection rate of stapes malformation was 50.00% in transverse and coronal position and 78.13% in axial position. The detection rate of stapes in axial position was higher than that in standardized reconstruction, and the difference was statistically significant(P=0.016).The detection rates of the abnormal stapes anterior crus, posterior crus and stapes head were 52.38%(11/21),52.63%(10/19) and 37.50%(6/16), respectively.Axialdetection rates of stapes were 90.48%(19/21),94.74%(18/19) and75. 00%( 12/16),respectively. There were significant differences in the detection rates of the above three structural abnormalities between the two methods( P = 0. 006,0. 003,0. 033). In 14 fixed stapes malformations,no positive results were found in routine recombination and axial position of stapes. Conclusion MSCT has limited ability to evaluate the floor of the stapes,and axial recombination of the stapes can improve the ability to detect the abnormal structures of the stapes.
