原发性囊腔型肺癌的CT诊断13例诊断分析

目的探讨原发性囊腔型肺癌的CT表现,以提高对本病的诊断准确率。方法回顾性分析经病理证实的13例原发性囊腔型肺癌的CT表现。结果13例囊腔型肺癌均为单发病例,左肺5例,右肺8例,其中10例腺癌,2例鳞癌,1例小细胞肺癌。病灶边缘分叶、毛刺分别占61.5%、76.9%;囊内壁毛糙、厚薄不均分别占84.6%、92.3%;囊内分隔占61.5%;囊腔内壁结节占53.8%;病变周围胸膜凹陷和血管集束分别占69.2%、84.6%。8例增强检查病例中6例轻度强化,2例中度强化。动态随访病例中,1例壁结节增大,1例囊腔增大,2例囊腔及实性成分均增大。结论囊腔内壁毛糙,囊内分隔及壁结节是有助于囊腔型肺癌诊断的特征性征象,有助于其鉴别诊断。

Diagnosis and Analysis of 13 Cases of Primary Cystic Type Lung Cancer by CT Diagnosis

Objective To investigate the CT manifestations of primary cystic lung cancer in order to improve the diagnostic accuracy of the disease.Methods The CT findings of 13 patients with pathologically confirmed primary cystic lung cancer were retrospectively analyzed.Results The 13 cases of cystic lung cancer were single cases,5 in the left lung and 8 in the right lung,of which 10 were adenocarcinoma,2 were squamous cell carcinoma,and 1 was small cell lung cancer.Lesion margins and burrs accounted for 61.5%and 76.9%,respectively;rough and uneven thickness of the inner wall of the capsule accounted for 84.6%and 92.3%,respectively;intrasaccular separation accounted for 61.5%;nodules of the inner wall of the capsule accounted for 53.8%;Vascular bundles accounted for 69.2%and 84.6%,respectively.Six of the eight enhanced examination cases were mildly enhanced and two were moderately enhanced.In dynamic follow-up cases,one case had an enlarged wall nodule,one had a larger cyst,and two had larger cysts and solid components.Conclusion The inner wall of the cystic cavity is rough,the cystic division and the wall nodules are the characteristic signs that are helpful for the diagnosis of cystic lung cancer,which is helpful for its differential diagnosis.
