大剂量热碘油TACE联合射频消融术治疗消化道癌肝转移

目的评价大剂量热碘油肝动脉化疗栓塞术(TACE)联合射频消融术(RFA)治疗消化道癌肝转移的临床疗效。方法将96例消化道癌肝转移患者分为两组,48例(联合组)采用大剂量热碘油TACE联合RFA治疗;48例(对照组)采用常温碘油TACE治疗。结果联合组有效率为75%(36/48),对照组有效率为33%(16/48),两组间差异有统计学意义。两组术后肝功能变化无统计学意义。术后6、12、18、24个月生存率联合组分别为100%、85%、56%、27%,对照组分别为90%、53%、19%、8%。结论大剂量热碘油TACE联合RFA可以有效控制肝转移癌患者肝内病变的进展,延长患者生存期,而肝功能无明显的损害。

Large dose heat lipiodol TACE combined with radiofrequency ablation in treatment of hepatic metastases of digestive tract cancer

Objective To study the clinic effects of large dose heat lipiodol transcatheter arterial chemoembolization （TACE） com- bined with radiofrequency ablation （RFA） in the treatment of hepatic metastases of digestive tract. Methods A total of 96 patients with hepatic metastases of digestive tract cancer were divided into two groups. Trial group （ n = 48 ） was treated by large dose heat lipiodol TACE combined with RFA and control group （ n = 48 ） treated by normal temperature lipiodol TACE. Results Effective rate was 75% （36/48） in the trial group and 33% （16/48） in the control group, between which there was a statistically signifi- cant difference. The changes in liver function after operation were of no statistically significant difference between the two groups. Postoperative survival rates of 6, ]2, 18 and 24 months were 100% , 85% , 56% and 27% in the trial group and 90% , 53% , 19% and 8% in the control group, respectively. Conclusion large dose heat lipiodol TACE plus RFA helps to control the patho- logical development inside liver in the patients with hepatic metastasis and therefore prolong their survival period without obvious damage to liver function.
