原发性腹膜后肿瘤19例临床分析

目的 探讨原发性腹膜后肿瘤的临床表现、诊断及外科治疗方法,提高患者生存率.方法 回顾性分析1991年3月至2011年3月我院收治的原发性腹膜后肿瘤19例,其中良性肿瘤5例,恶性肿瘤14例,术后均经病理学检查证实,就其临床表现,影像学检查,病理学检查,手术治疗,术后生存及复发等方面进行分析.结果 本组良性肿瘤完全切除率为80％ (4/5),恶性肿瘤完全切除率为57.1％ (8/14),同时合并邻近脏器切除5例,为35.6％ (5/14).超声、CT及MR的诊断率分别为89.5％、93.3％及100％.结论 原发性腹膜后肿瘤手术治疗是主要手段,并尽可能做完整切除,复发者条件允许进行再次手术.超声联合CT检查为原发性腹膜后肿瘤早期的主要检查方法,MR检查阳性率最高.

Clinical Analysis of 19 Cases of Primary Retroperitoneal Tumor

Objective To discuss the clinical manifestations,diagnosis and surgical treatment of primary retroperitoneal tumor(PRT),to increase the survival rate.Methods The 19 cases of primary retroperitoneal tumor had been received and treated in our hospital since March 1991 to March 2011,and the clinical data were analyzed retrospectively.Among them,5 cases were benign tumors and 14 malignant ones.All of them had been diagnosed pathologically.The clinical manifestations,imaging findings,pathological types,surgical treatment,survival rate and recurrent rate were analyzed.Results The high complete excision rate of benign tumor was 80%(4/5);the rate of malignant tumor was 57.1%(8/14).Meantime,there were 5 cases of adjacent organs resection,the rate was 35.6%(5/14).The diagnosis rates of B-ultrasonic,CT and MR were 89.5%,93.3% and 100%.Conclusion Surgical therapy is treated as a main surgical strategy for the treatment of PRT,and total removal of tumors should be done if possible.If the conditions allow,re-operation is the first choice for recurrent PRT.B-ultrasonic and CT examination play an important role in earlier diagnosis of PRT,and MT examination has the highest positive rate.
