腹腔镜子宫腺肌瘤切除术临床应用研究

目的探讨腹腔镜下子宫腺肌瘤切除术的临床疗效。方法回顾性分析本院59例接受腹腔镜子宫腺肌瘤切除术患者的临床资料、诊疗经过及术后疗效。结果59例患者中,34例患者有不同程度痛经,其中重度痛经17例,中度痛经9例,轻度痛经8例;月经量增多31例,其中伴月经紊乱10例,12例无明显症状。平均手术时间（62.4±11.5）min,术中中位出血量50mL,术后平均肛门排气时间（25.2±9.8）h,术后平均停留尿管时间（8.0±4.6）h,术后平均住院时间（4.8±1.1）d,无术中、术后并发症发生。术后随访6～70个月,痛经缓解率73.5%（25/34）,月经改善率90.3%（28/31）。结论腹腔镜子宫腺肌瘤切除术具有创伤小、出血少、术后恢复快、临床疗效良好等优点,对于年轻、未生育或要求保留子宫者值得应用。

Clinical study on laparoscopic Surgery of adenomyoma excision

Objective To evaluate the efficiency of laparoscopic surgery of adenomyoma excision in treatment of adenomyoma. Methods A retrospectively analysis was carried out on 59 cases with adenomyoma and treated by laparoscopic surgery of adenomyoma excision. Results Thirty-four patients suffered for dysmenorrhea in different degrees in total of 59 cases, 17 for severe, 9 for moderate and 8 for mild. Thirty-one patients were menorrhagia, among them, 10 patients suffered from menstrual disorders. Twelve patients had no obvious clinical symptoms. The mean operation time was （62.4± 11.5）min, and the moderate bleeding during operation was50 mL. The mean resume time of digestive system and bladder was （25.2±9. 8）hours and mean time of ureter lien was（8. 0±4. 6）hours respectively. The mean hospitalized time after operation was（4. 8 ±1.1） days. All operations were performed laparoscopiclly without complication. The postoperative follow-up which time range from 6 to 70 months indicates that the relief rate of dysmenorrheal was 73. 5% （25/34 ） and improved rate of menstrual disorders was 90. 3% （28/31）. Conclusion The results shown that there are mini-trauma, less bleeding, rapid recovery, and good clinical therapy which the laparoscopic surgery of adenomyoma excision. It could be recommended for those nulliparous young patients with desire of an integrated uterus.
