胶原酶2种盘外注射法治疗腰椎间盘突出症

目的：观察２种盘外注射胶原酶方法治疗腰椎间盘突出症（腰突症）的临床疗效。方法：８１例腰突症病人接受２种胶原酶１２００Ｕ盘外注射法治疗。一种是硬膜外腔后间隙法，另一种是硬膜外腔前间隙法。采用改进的日本骨科学会制定的腰腿痛疗效标准评分。结果：硬膜外腔后间隙法术后７ｄ的病例优良率为３４％，总有效率为７３％。平均随访４．８±２．９ｍｏ的优良率为６３％，总有效率为８６％。硬膜外腔前间隙法术后７ｄ的病例优良率为４１％，总有效率为７５％。平均随访３．０±１．０ｍｏ时的优良率为６６％，总有效率为８８％。结论：２种盘外注射胶原酶治疗腰突症都是有效的方法。

Two methods of extra_disc injection of collagenase for lumbar disc herniation

AIM: To assess the curative effects with 2 methods of extra_disc injection of collagenase for lumbar disc herniation. METHODS: Two methods of extra_disc collagenase injections were performed in 81 patients of lumbar disc herniation. One was posterior epidural space injection, and the other was anterior epidural space injection. All patients were followed up 2 months later. The modified Japanese Orthopedics Association's assessment of treatment for low back pain was used. RESULTS: Seven days after the posterior epidural space injection, 34% of the 49 patients obtained excellent or good results, the total effective rate was 73%; With an average of 4.8±2.9 mo follow_up, the good or excellent results were noted in 63%, the total effective rate was 86%. Seven days after the anterior epidural space injection, 41% of the 32 patients obtained excellent or good results, the total effective rate was 75%; With an average of 3.0±1.0 mo follow_up, the good or excellent results were noted in 66%, the total effective rate was 88%. CONCLUSION: The extra_disc injection of collagenase is an effective method for the treatment of lumbar disc herniation.
