藻酸双酯钠治疗急性脑梗死的疗效和4种血浆生物分子治疗前后比较

摘    要:目的 :研究藻酸双酯钠 (PSS)治疗急性脑梗死 (ACI)的临床疗效和血浆生物分子的变化。方法 :随机选择临床确诊的ACI 98例 ,分成 3组 ,分别给予PSS ,胰岛素和三七总皂苷治疗。观察临床疗效和检测治疗前后病人的血脂、血糖 (Glu)、内皮素(ET 1)、P 选择素 (P selec)、血管内皮生长因子(VEGF)和血小板活化因子 (PAF)的变化。结果 :3组总有效率分别为 94 % ,96 % ,96 % ,经Ridit分析 ,疗效比较差异无显著意义 (P >0 .0 5 )。治疗后PSS组TG ,TC ,Glu ,ET 1,P selec和PAF值分别为(1.1± 0 .4 )mmol·L- 1,(4.2± 0 .7)mmol·L- 1,(7.2± 2 .3)mmol·L- 1,(33± 4 )ng·L- 1,(2 9± 4 )μg·L- 1和 (6 0± 14 ) μg·L- 1较治疗前明显降低 ,差异有显著和非常显著意义 (P <0 .0 5和 0 .0 1) ;PSS组与RI组或三七总皂苷组血糖组间比较 ,差异有非常显著意义 (P <0 .0 1)。结论 :PSS治疗ACI的临床疗效与胰岛素 ,三七总皂苷相似 ,但对血浆生物分子学的改变则有显著差别。

Efficacy of polysaccharide sulfate for acute cerebral infarction and comparison of four items plasma biomoleculars in before and after treatment
Abstract:AIM: To study the clinical efficacy of polysaccharide sulfate(PSS) for acute cerebral infarction(ACI) , and the changes of plasma biomoleculars. METHODS: Ninety eight patients with ACI were randomly collected. They were divided into three groups. PSS group of forty seven patients were treated with PSS. Regular insulin (RI) group of twenty six patients were treated with RI. Panax notoginseng saporins group of twenty five patients were treated with Panax notoginseng saporins. The clinical effects were observed.The plasma levels of blood glucose,lipids, endothelin 1 (ET 1) , P selectin(P selec), Platelet activating factor(PAF) , and vascular endothelial growth factor (VEGF) were all measured together,and compared among three groups before and after treatment. RESULTS: The total effective rate was 94 % , 96 % and 96 % in the PSS group , RI group, and Panax notoginseng saporins group , respectively .There was no difference in clinical efficacy among three groups by Ridit analysis after treatment ( P>0.05 ). After treatment the value of TG , TC , Glu , ET 1 , P selec , and PAF in PSS group was ( 1.1±0.4 ) mmol·L -1 , (4.2±0.7) mmol·L -1 , (7.2±2.3) mmol·L -1 , (33±4) ng·L -1 , (29±4) μg·L -1 , and (60±14) μg·L -1 , respectively; and it had marked difference compared with that before treatment (P<0.05 or P<0.01 ). PSS group had marked difference compared with RI group or Panax notoginseng saporins group (P< 0.01 ). CONCLUSION: The clinical efficacy of PSS is as similar as that of RI and Panax notoginseng saporins, but there is marked difference in the plasma biomoleculars.
