| 摘 要： | 目的探讨腹腔镜胆囊切除术(LC)时CO2气腹对老年患者动脉血气和血流动力学的影响。方法选择择期行LC手术的老年患者60例,全部采用气管插管全身麻醉。分别于麻醉后CO2气腹前及其后10、30min、放气后30min时采桡动脉血进行血气分析,记录全程血压、脉搏、心率、血氧饱和度。结果与气腹前比,PaCO2在气腹后10、30min、放气后30min均明显增高(P<0.05)。pH的变化则与其相反,与气腹前比,在气腹后10、30min、放气后30min均明显降低(P<0.05)。PaO2、SaO2在气腹前后无明显变化(P>0.05)。BP、HR等在气腹前后无明显变化(P>0.05)。结论LC手术中CO2气腹对老年患者动脉血气有一定的影响,但pH、PaCO2值的变化在正常范围之内,能维持血流动力学稳定。

| Abstract: | Objective To study the effect of Carbon Dioxide Insufflation on arterial blood gas and hemodynamics in the elderly. Method Sixty gerontal patients undergoing laparoscopic cholecystectomy during endo-trachea general anesthesia were randomly selected. Artery blood samples were drawn to measure pH,PaCO2,PaO2 and SaO2. The vital signs were monitored simultaneously. Results PaCO2 was significantly increased on the 10th min, 30th min after the insufflation and 30th min after the deflation, compared with pre-inflation value (P<0.05);but pH wassignificantly decreased; PaO2,SaO2,blood pressure and heart rate remained un-changed (P>0.05).Conclusion Carbon Dioxide Insufflation can obviously influence on arterial blood gas in the aged, but keep stable hemodynamics. |
