银杏叶提取物联合泼尼松治疗对特发性肺间质纤维化患者血浆TNF-α、IL-10的影响

摘    要:目的 观察银杏叶提取物联合泼尼松治疗对特发性肺间质纤维化患者血浆肿瘤坏死因子-α(TNF-α)及白细胞介素10(IL-10)的影响,从细胞因子角度探讨银杏叶提取物治疗特发性肺间质纤维化的机制.方法 将70例特发性肺间质纤维化随机分为两组,银杏叶提取物+泼尼松组(观察组)32例、泼尼松组(对照组)38例,分别于治疗前1天及治疗后4、8周清晨采集两组空腹静脉血并处理、留存标本,对各标本中的TNF-α,IL-10浓度进行定量检测.结果 两组治疗前各检测指标差异均无统计学意义(P>0.05);治疗后4周两组各指标均较前升高,同期相比观察组TNF-α水平较对照组低,但差异无统计学意义(P>0.05),IL-10水平亦较对照组低,差异有统计学意义(P<0.01);治疗后8周,两组各指标较前进一步升高,同期相比,观察组TNF-α、IL-10水平均较对照组低,差异均有统计学意义(P<0.05,P<0.01).结论 银杏叶提取物联合泼尼松较单独应用泼尼松能更好地治疗特发性肺间质纤维化,其机制可能与抑制TNF-α、IL-10的产生有关.

Abstract:Objective To evaluate the combination effects of EGB and Prednisone on TNF-α and IL-10 obtained from patients with idiopathic pulmonary fibrosis and explain their mechanisms at molecular biological level. Methods 70 patients with IPF were randomly divided into two groups: EGB + Prednisone group (therapy group, n = 32) , Prednisone group (control group, n = 38). The level of TNF-α and IL-10 of the patients'blood plasma were detected with ELISA method at three time-points ( 1 day before therapy, 4 and 8 weeks after therapy) , and all samples were processed and then stored. Results The comparison of the levels of TNF-α and IL-10 in plasma among two groups had no significant difference before therapy (P 〉 0.05 ). The indexes increased progressively within the first 4 weeks after therapy, and the level of TNF-α in plasma of therapy group was lower than that of control group at the same time ( P 〉 0.05 ). The concentration of IL-10 was lower than that of control group (P 〈0.01 ). The indexes increased 8 weeks after therapy. The level of TNF-α in plasma of therapy group was lower than that of control group at this time (P 〈 0.05 ) , and IL-10 was lower than that of control group (P 〈 0.01 ). Conclusion Combination of EGB and prednisone may be a better approach to treat IPF patients than Prednisone alone, and this may be related with its effect to bring down the level of TNF-α and IL- 10 in patient's plasma.
