摘    要:目的 探讨64排螺旋CT血管成像(64-SCTA)对脑动脉瘤的诊断价值.方法 收集辽宁中医药大学附属医院2013年3月-2016年1月收治的62例脑动脉瘤患者的临床资料,所有患者入院后均接受64-SCTA和数字减影血管造影(DSA)检查,以DSA结果作为金标准,分析64-SCTA对脑动脉瘤的诊断价值.结果 DSA共检出动脉瘤65个,64-SCTA检出动脉瘤62个,符合率为95.38%,两者脑动脉瘤检出率相近(P >0.05);64-SCTA与DSA测定动脉瘤瘤体长轴、瘤体短轴及瘤颈宽度一致(P>0.05);DSA、64-SCTA瘤体定位评分比较差异无统计学意义(P>0.05),但64-SCTA对瘤体形态、三维关系显示质量评分优于DSA(P <0.05).结论 64-SCTA与DSA诊断脑动脉瘤准确率相近,且安全、无创,配合后处理技术,可显示瘤体形态、大小、空间关系,有助于手术方案的确立.

Abstract:Objective To investigate value of 64-slice spiral CT angiography (64-SCTA) in diagnosis of patients with cerebral aneurysms.Methods Clinical data of 62 patients with cerebral aneurysm admitted between March 2013 and January 2016 was collected.All patients underwent 64-SCTA and digital subtraction angiography (DSA) examinations.Value of 64-SCTA in diagnosis of cerebral aneurysms was analyzed by using DSA as the golden standard.Results A total of 65 aneurysms were detected by DSA,and 62 aneurysms were detected by 64-SCTA,and the coincidence rate was 95.38%,and the detection rates of both methods for cerebral aneurysms were similar (P > 0.05);64-SCTA and DSA were consistent in determination of long and short axes of aneurysms and width of aneurysm neck (P > 0.05);differences in tumor location scores of DSA and 64-SCTA were not statistically significant (P > 0.05),but the quality of 64-SCTA was superior to DSA in displaying morphology and 3D relationship of aneurysms (P < 0.05).Conclusion Accuracy rates of 64-SCTA and DSA in diagnosis of cerebral aneurysms are similar with safe and non invasive.Combined with post-processing techniques,they can display shape,size and spatial relationship of the tumors,which is helpful to the establishment of surgical procedures.
