多重耐药铜绿假单胞菌β-内酰胺类相关耐药基因研究

摘    要:目的 调查我院2005年临床分离多重耐药铜绿假单胞菌(PA)的耐药性分析并探讨其耐药基因的存在状况。方法对31株临床分离的铜绿假单胞菌用纸片扩散法检测其药敏表型,采用聚合酶链反应(PCR)法检测β-内酰胺酶编码基因和外膜通道蛋白OprD2基因,并与相关药敏表型比较。结果31株铜绿假单胞菌对16种抗菌药物的耐药率为48.4%~100%;β-内酰胺酶编码基因PER、VIM、CARB、IMP、TEM、OXA-10的检出率分别为6.4%、3.2%、3.2%、3.2%、3.2%、3.2%,未检出GES、SHV、VER基因,31株外膜通道蛋白OprD2基因缺失,检出率为100%。结论本院多重耐药铜绿假单胞菌对β-内酰胺酶类抗生素的耐药主要与外膜通道蛋白OprD2基因缺失有关。

Study on the resistant genes of β-lactam antibiotics of the multi-drug resistant mechanism of pseudomonas aeruginosa

Abstract:Objective To survey the distribution of aminoglycoside-modffying enzyme genes in multidrug-resistant pseudomonas aeruginosa isolated in our hospital in 2005. Methods The antibiotic susceptibility of 31 different antibiotics of P. aeruginosa was tested by K-B method, and 31 multi-drug resistant strains were screened ; β-lactamase genes and the outer membrane protein OprD2 gene were detected by polymerase chain reaction(PCR). Results Resistant rates of 31 strains of P. aeruginas against 16 kinds of antibiotic ranged from 48.4%to 100% ;the detection rate of β-1actamase gene PER,VIM,DHA,IMP,TEM and OXA-10 was 6. 4%, 3.2%,3.2% ,3.2% ,3.2% and 3.2% respectively,and none of the 31 strains possessed genes of GES,SHV and VER,31 isolate of PA lost the outer membrane protein OprD2 gene. Conclusion Resistance to β-lactam compounds in P. aeruginas of our hospital is related to lost the outer membrane protein.
