应用红外热像技术测量蛛网膜下腔麻醉平面的研究

摘    要:【摘要】 目的: 探讨应用红外热像技术测量蛛网膜下腔麻醉平面的临床价值。方法:对24例择期行下腹部及下肢手术患者应用红外热像技术采集蛛网膜下腔麻醉前、后红外热像图、定性、定量进行分析,分析热图温度平面变化、并同针刺法测定的痛觉阻滞平面法进行比较。结果:红外热像仪测量的最高温度平面高于针刺法测定的痛觉阻滞平面,痛觉阻滞平面固定后为T9-S1,红外热像仪测量最高温度平面为T12-L5,二者相差2个脊神经节段;蛛网膜下腔神经被阻滞后红外热像图色码温区发生了变化,尤其是L5-S1所支配的神经平面色码温区变化显著。结论:可以间接通过用红外热像技术测量痛觉阻滞平面、红外热像技术用于蛛网膜下腔麻醉平面的测量具有一定的临床实用价值。

Abstract:Objective:To evaluate the clinical value of infrared thermal imaging technique in detecting the block height of anesthesia.Methods: Infrared thermal images were collected from 24 patients undergoing lower abdominal and lower limb surgery before and after spinal anesthesia.Changes in thermal levels were analysed qualitatively and quantitatively.Results: Levels of sensory block obtained with infrared imaging technique were significantly higher at 10,15,20,25,45 and 60 min than that of 5 min(P < 0.05).Levels of sensory analgesia were also significantly higher at 20,25 and 45 min than that of 10 min(P < 0.05).There were no significant differences in levels of sensory block between 15,60 and 10 min(P > 0.05).The highest level of sensory block was at 25 min.The thermal levels were higher after anesthesia compared with that before anes thesia.The average thermal levels were significantly increased at 25 min,45 min and 60 min after anesthesia compared with that of 5 min(P < 0.05).There were no significant changes in average thermal levels 10 min,15 min and 20 min after anesthe sia(P > 0.05).Conclusion: The infrared thermal imaging technique can be used to measure the spinal anesthetic block lev els indirectly,which suggests some clinical application value.
