厄贝沙坦氢氯噻嗪治疗老年重症心力衰竭效果观察

目的观察厄贝沙坦氢氯噻嗪治疗老年重症心力衰竭的临床效果。方法选择2017年1月-2018年12月河北省河间市人民医院收治的老年重症心力衰竭患者130例,采用随机数字表法分为观察组和对照组各65例,对照组采用常规治疗,观察组在对照组治疗的基础上联合厄贝沙坦氢氯噻嗪治疗。比较2组治疗效果、治疗前后心功能指标左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室短轴收缩末内径(LVESD)]及脑钠肽(BNP)、心型脂肪酸结合蛋白(H-FABP)水平。结果观察组患者总有效率为98.46%,高于对照组的76.92%(P<0.05)。治疗后,2组患者LVEF高于治疗前,LVEDD、LVESD低于治疗前,且观察组变化幅度大于对照组(P均<0.05)。治疗后,2组患者BNP、H-FABP均低于治疗前,且观察组低于对照组(P均<0.05)。结论厄贝沙坦氢氯噻嗪治疗老年重症心力衰竭效果较好,可显著改善心功能,具有较高临床应用价值。

Effect observation of irbesartan hydrochlorothiazide in the treatment of elderly patients with severe heart failure

Objective To observe the clinical effect of irbesartan hydrochlorothiazide in the treatment of senile severe heart failure.Methods 130 cases of elderly patients with severe heart failure treated in the People’s Hospital of Hejian City from January 2017 to December 2018 were selected.Random number table method was used to divide the observation group and control group,with 65 cases each.The control group was treated with conventional treatment,and the observation group was combined with irbesartan hydrochlorothiazide on the basis of treatment in the control group.Compared the treatment effects,cardiac function indexes,brain natriuretic peptide(BNP),and cardiac fatty acid binding protein(H-FABP)levels of the two groups before and after treatment.Cardiac function indicators included left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),and left ventricular short-axis end-systolic diameter(LVESD).Results The total effective rate of patients in the observation group was 98.46%,which was higher than 76.92%in the control group(P<0.05).After treatment,the LVEF of patients in two group was higher than before treatment,and LVEDD and LVESD were lower than before treatment,and the changes in the observation group were greater than those in the control group(P<0.05).After treatment,the BNP and H-FABP of the two groups were lower than before treatment,and the observation group was lower than the control group(P<0.05).Conclusion Irbesartan hydrochlorothiazide has a better effect on elderly patients with severe heart failure,can significantly improve cardiac function,and has high clinical application value.
