高尿酸血症合并高血压临床分析

目的探讨高尿酸血症与高血压及其他代谢综合征成分的关系。方法以80例单纯高尿酸血症患者(HU组)为对照,对70例高尿酸血症合并高血压患者(HU/HT组)进行临床分析。分别测量两组的身高、体重、血压、腰围、臀围、空腹血糖(FPG)、血尿酸(UA)、血尿素氮(BUN)、肌酐(CRE)、胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)并进行比较。结果①HU/HT组UA、TG、TC、FPG均高于HU组(P<0.05),提示高血压可以使血尿酸水平升高,高尿酸合并高血压时进一步加重糖脂代谢紊乱;②HU/HT组体重指数(BMI)、腰臀比(WHR)、腰围、臀围均显著高于HU组(P<0.01),提示肥胖,尤其是中心性肥胖是高血压合并高尿酸血症的危险因素;③HU/HT组中代谢综合征的患病率高于HU组(P<0.01),提示高尿酸血症合并高血压与代谢综合征关系更为密切。结论肥胖、高血压是高尿酸的危险因素;高尿酸血症合并高血压时与糖脂代谢紊乱等代谢综合征(MS)成分关系更为密切。

Clinical observation of hyperuricemic patients complicating with hypertension

Objective To explore the relationship of hyperuricemia(HU) and hypertension(HT) and metabolism syndrome.Methods Clinical analysis of 80 cases of HU and 70 cases of HU/HT were conducted.The body height,body weight,blood pressure,waistline,hip circumference,fasting plasma glucose(FPG),blood uric acid(UA),blood urea nitrogen(BUN),creatinine(CRE),cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol c(HDL-C) of the two groups were measured.Results (1)UA,TG,TC,and FPG of HU/HT group were higher than those of HU group(P<0.05),prompting hypertension could increase the level of blood uric acid and when hyperuricemia combined hypertension,metabolic disorder became severer.(2)Body mass index,waist hip ratio(WHR),waistline,hip circumference of HU/HT group were obviously higher than those of HU group(P<0.01),which hinted obesity,especially central obesity was a risk factor of hyperuricemia complicating with hypertension.(3) Compared with HU group,prevalence of metabolism syndrome in HU/HT group was higher (P<0.01),indicating that the relationship of hyperuricemia complicating with hypertension and metabolism syndrome was more intimate than pure hyperuricemia.Conlusion Obesity and hypertension are risk factors of hyperuricemia.When complicating with hypertension,metabolism disorder becomes severer.
