2013年某院抗菌药物使用强度分析

目的分析2013年某院住院患者抗菌药物使用强度,为该院设定下年度各科室抗菌药物使用强度控制指标提供依据。方法采用Excel 2003进行统计,以限定日剂量（DDD）的方法,计算抗菌药物用药频度（DDDs）、使用强度（AUD）、住院人均用药频度等,并进行分析。结果 2013年某院住院患者抗菌药物使用强度为50.41 DDD,使用强度排名前3位的抗菌药物类别分别为第二代头孢菌素、第三代头孢菌素、第一代头孢菌素,使用强度排名前3位的科室分别是外科ICU、内科ICU、呼吸内科,住院人均用药频度排名前3位的科室分别为内科ICU、呼吸内科、外科ICU。结论某院在抗菌药物合理应用方面仍存在不少问题,如用药过度、用药档次较高、注射剂使用过多、部分品种使用过于集中等,需要进一步加强管理。科学设定各科室使用强度控制指标,应注意使用强度、用药频度、住院人均用药频度、平均住院天数等指标的联动。

Analysis of Antibiotics Use Density in A Hospital in 2013

Objective To provide basis for making control indicators of antibiotics use density（ AUD） for each clinical department through analysis AUD of inpatient in the hospital in 2013. Methods The data were calculated and analyzed through DDDs,AUD,DDDs of inpatient with Excel 2003. Results The AUC of inpatient was 50. 41 DDD in 2013. The top 3 AUC were 2nd generation cephalosporins,3rd generation cephalosporins,1st generation cephalosporins. The top 3clinical department of AUC were intensive care unit（ ICU）,respiratory medicine,coronary care unit（ CCU）. The top 3clinical department of per capita defined daily dose system（ DDDs） were CCU,respiratory medicine,and ICU.Conclusion There were still quite a few problems in the application of antibacterial medicine,we should enhance the managements,such as over-medication,high class of drug,excessive use of injection,concentration on some varieties.Furthermore,scientific indicators should be made to control the AUC for each clinical department,and focus on the association with all indicators,as AUC,DDDs,per capita DDDs of inpatients,the average hospitalization days.
