临床药师参与的血药浓度危急值管理实践

血药浓度危急值管理具有其特殊性,既是医院危急值管理的一部分,也是规范临床用药、保障患者用药安全的重要手段。因此,医院应重视血药浓度危急值的管理,针对临床检测发现的血药浓度危急值,应及时反馈、沟通处理。在医疗改革日益深化的今天,专科临床药师是医院药学技术服务转型的主力军。本院从2013年起,临床药师、检测药师和医护人员共同参与、逐步探索,建立了患者的血药浓度危急值管理和临床干预体系,对临床检测的血药浓度危急值进行预警、跟踪和临床干预。所建立的血药浓度危急值管理模式,显著提高了我院血药浓度危急值的上报率,同时也降低了危急值患者药品不良反应的发生率。

A Novel Management Practice for Urgent Value of Plasma Drug Concentration with the Participation of Clinical Pharmacists

The management of urgent value for therapeutic drug monitoring is not only part of urgent value management in hospitals, but also an important tool for ensuring reasonable drug usage and patient"s safety. Therefore, it is necessary for hospitals to pay attention to urgent value in therapeutic drug monitoring, and further clinical intervention and response is needed. With the new medical reform, clinical pharmacists are playing more and more important roles in providing pharmaceutical services. A novel urgent value management and clinical intervention system for therapeutic drug monitoring which includes the participation of clinical pharmacists, technicians and medical staffs has been established in our hospital since 2013. It has improved the reporting rate of urgent values, and cut down the incidence rate of adverse drug reactions significantly.
