人工股骨头置换术治疗老年股骨粗隆间骨折的疗效分析

目的探讨人工股骨头置换术治疗老年股骨粗隆间骨折的临床疗效。方法将125例老年股骨粗隆间骨折患者根据手术方法的不同分为观察组(65例)和对照组(60例),观察组采用人工股骨头置换术治疗,对照组采用PFNA内固定治疗。结果对照组在手术时间、术中出血量均优于观察组(P<0.05),但观察组在术后下地活动时间及髋关节Harris评分优于对照组(P<0.05);观察组术后近期并发症发生率为16.9%,高于对照组的5.0%(P<0.05),观察组长期并发症发生率为3.1%,明显低于对照组的31.7%(P<0.05)。结论采用人工股骨头置换术治疗老年股骨粗隆间骨折能够使老年患者卧床时间缩短,尽早进行康复功能锻炼,促进髋关节功能的恢复,降低术后长期并发症的发生率,提高患者生活质量。

Analysis of the Curative Effect of Artificial Femoral Head Replacement for Senile Femoral Intertrochanteric Fracture

Objective To study the curative effect of artificial femoral head replacement for senile femoral intertrochanteric fracture. Methods 125 cases of senile femoral intertrochanteric fracture were divided into observation group （65 cases） and control group （60 cases） according to the different surgical methods. The artificial femoral head replacement was used in the observation group, while PFNA internal fixation was used in the control group. Results The operation time and intraoperative blood loss of the control group were superior to the observation group （P＆lt;0.05）, but the postoperative exercise time and the Harris score of the hip joint of the observation group were superior to the control group （P＆lt;0.05）;the postoperative recent complication rate of the observation group was 16.9%, which was higher than the control group （5.0%） （P＆lt;0.05）. The long-term complication rate of the observation group was 3.1%, which was significantly lower than the control group （31.7%） （P＆lt;0.05）. Conclusion The treatment of artificial femoral head replacement on senile femoral intertrochanteric fracture can make the bedridden time of the elderly patient to be shorter, do rehabilitation exercise as soon as possible, promote the recovery of hip joint function, reduce the incidence of postoperative long-term complications, and improve the quality of life.
