重症医学科患者的来源及病种分类

目的分析近3年重症医学科患者来源及病种分类，探讨我院ICU医务人员配备问题及应该努力的方向。方法采用回顾分析的方法进行分析。结果3年来我院重症医学（ICU）科共收治2048例患者。患者来源构成为急诊转入1134例．（45．41％）、会诊转入511例（43．64％）、术后转入373例（12．60％）、外院转入30例（23．33％），会诊、急诊、术后及外院转入组之间两两比较，会诊与急诊转入组在病死率方面差异不具有显著性，而术后及外院转入组与前二者相比差异有统计学意义，P〈0．05。对患者依据病因及病例数分类排序为心脑血管病类769例（38．86％）、其余依次为中毒及严重创伤437例（2134％）、多器官功能不全323例（15．77％）、严重感染271例（13．23％）、恶性肿瘤200例（9．77％）、其他21例（1．03％）。结论加强重症医学科与其他各科间的合作关系以及加强科室自身业务素质仍是今后ICU科应该努力的方向。

Analysis of Sources of Patients and Disease Category in an ICU

Objective To explore the assignments and the efforts direction of medical staff in our hospital＇s ICU. Methods The retrospective method for analysis. Results In recent 3 years, 2048 ceases were accepted by ICU. The source of cases include four groups： 1134 cases （45.41%） in group transferred from emergency, 511 cases （43.64%） in group transferred from consultation 373 cases （12.60%） in group transferred from post-operation and 30cases （23.33%） in group transferred from other hospitals. Th~ in-hospital mortality in groups transferred from post -operation and other hospitals were significantly lower than that in group transferred from emergency or that in group transferred from consultation （P〈0.05）. The disease category include six groups： 769 ceases （38.86%） in group of cardio-cerebrovascular disease, 437 cases （21.34%） in group of poisoning and severe trauma, 323 eases （15.77%） in group of multiple organ dysfunction, 271 cases （13.23%） in group of severe infection, 200 cases （9.77%） in group of malignant tumor, and 21cases （1.03%） in the other group. Conclusions Strenghtening the cooperative relationship between ICU and other departments and improving in our professional quality in critical medicine are still the efforts direction for ICU.
