胰岛素样生长因子-Ⅰ与原发性高血压

目的探讨胰岛素样生长因子-Ⅰ(IGF-Ⅰ)与原发性高血压及高血压左心室肥厚(LVH)的关系,为高血压的诊断和治疗提供新思路。方法对26名正常人及74例原发性高血压患者均行超声心动图检查,并用放射免疫法测定血清IGF-Ⅰ水平。74例原发性高血压患者降压治疗3个月后测定血压与血清IGF-Ⅰ水平。结果①原发性高血压患者血清IGF-Ⅰ水平(298±61)ng/ml明显高于正常对照组(178±51)ng/ml,P<0.05。②原发性高血压组血压1～3级间血清IGF-Ⅰ水平比较差异有统计学意义分别为(238±54)ng/ml,(295±79)ng/ml,(376±62)ng/ml,P<0.05]。③原发性高血压伴LVH者血清IGF-Ⅰ水平(369±64)ng/ml]高于无LVH者(255±69)ng/ml],IGF-Ⅰ水平与左室重量指数(LVMI)中度正相关(r=0.52)。④原发性高血压组经药物治疗3个月后血压达标者其血清IGF-Ⅰ水平(241±59)ng/ml]显著低于血压未达标者(299±49)ng/ml],P<0.05。结论IGF-Ⅰ可能参与了原发性高血压的病理生理过程及LVH形成。

Insulin-like growth factor-Ⅰ and essential hypertension

Objective To investigate the relationship between insulin-like growth factor-I(IGF-I) and essential hypertension(EH) and left ventricular hypertrophy (LVH) . Methods In 26 normal individuals and 74 patients with EH,echocardiography were made,as well as the serum levels of IGF-I were measured using radioimmunoassay. Results ①IGF-I level (298±61)ng/ml in patients with EH was significantly higher than that (178±54)ng/ml in normal controls (P<0.05).②There was significant difference in the levels of IGF-I among the 3 clinical gradings of EH (238±54) ng/ml,(295±9) ng/ml,(376±62) ng/ml,P<0.05].③Levels of IGF-I was higher in EH with LVH(369±64) ng/ml than EH without LVH (255±69) ng/ml, and levels of IGF-I had positive correlation with left ventricular mass index (LVMI). Conclusion IGF-I is probably involved in the pathophysiological process of essential hypertension and left ventricular hypertension.
