摘    要:目的:探讨超声弹性成像在乳腺肿块诊断中的临床价值。方法:选取2017年1月~2018年1月在某院就诊行乳腺肿块手术切除并病检的患者60例,术前行常规超声与超声弹性成像检查。结果:病检结果显示良性肿块38例,恶性肿块22例。超声弹性成像检查的诊断准确率明显高于常规超声检查(P<0.05);恶性肿块超声弹性成像评分明显高于良性肿块(P<0.05);常规超声、弹性评分的曲线下面积(AUC)分别为0.665、0.868,差异有统计学意义(P<0.05);弹性应变率比值为2.1时诊断乳腺恶性肿块的敏感性75.42%,特异性89.78%,AUC为0.876。结论:弹性成像技术在乳腺实性肿块的诊断中具有一定的应用价值,可作为常规超声的辅助诊断,而弹性应变率比值可作为客观的超声诊断方法。

Abstract:Objective:To investigate the clinical value of ultrasound elastography in the diagnosis of breast masses.Methods:60 patients with breast mass resection and pathological examination from January 2017 to January 2018 were selected.Routine ultrasound and ultrasound elastography were performed before operation.Results:The pathological examination showed 38 cases of benign mass and 22 cases of malignant mass.The diagnostic accuracy of ultrasound elastography was significantly higher than that of conventional ultrasound(P<0.05);The score of ultrasound elastography for malignant masses was significantly higher than that of benign masses(P<0.05);The area under curve(AUC)of conventional ultrasound and elasticity score was 0.665 and 0.868,respectively,with statistical significance.The sensitivity,specificity and AUC were 75.42%,89.78%and 0.876 respectively when the elastic strain rate ratio was 2.1.Conclusion:Elastography technology has certain application value in the diagnosis of breast solid masses,and can be used as an assistant diagnosis of conventional ultrasound,while elastic strain rate ratio can be used as an objective ultrasound diagnosis method.
