摘    要:目的:观察加味四物汤(modified Siwu Tang concentrated solution,mSWT)对急性心肌梗死(acute myocardial infarction,AMI)大鼠血管新生、心室重构及心功能的影响。方法:结扎雄性SD大鼠冠状动脉左前降支制备AMI模型,假手术组(Sham组)、模型组灌胃给予0.9%氯化钠溶液,mSWT低剂量(mSWT-L)组和mSWT高剂量(mSWT-H)组分别灌胃给予mSWT 3 g·kg-1·d-1和6 g·kg-1·d-1处理35 d;彩色多普勒超声动态监测心功能变化;HE和Masson染色观察心肌组织病理学变化,免疫组化染色评价梗死区域CD31+细胞微血管密度;定量PCR检测VEGF、b-FGF mRNA水平。结果:mSWT-H组用药第7,14,35天EF、FS较模型组均显著升高(P<0.01),LVEDV和LVESV均明显低于模型组(P<0.01);mSWT-L组EF、FS第14,35天较模型组显著升高(P<0.01),LVEDV第7,14天和LVESV第7,14,35天显著低于模型组(P<0.01);用药第7天mSWT-H组该4项指标均较mSWT-L组显著改善(P<0.01)。mSWT-H、mSWT-L组较模型组心肌纤维化明显减轻,心肌组织病理学改善明显,心肌组织CD31+细胞微血管密度明显增加(P<0.01),VEGFb-FGF mRNA表达显著上调(P<0.01)。结论:mSWT上调心肌组织VEGFb-FGF表达,促进血管新生,改善心室重构,从而改善AMI大鼠心功能。

Abstract:OBJECTIVE To observe the effect of modified Si-Wu-Tang (mSWT) on the angiogenesis,ventricular remodeling and cardiac function of acute myocardial infarction (AMI) rats.METHODS AMI model was established by ligating the left anterior descending coronary artery of male SD rats.Sham group and model group were given normal saline by intragastric administration;mSWT-L group and mSWT-H group were given 3 and 6 g·kg-1·d-1by intragastric administration,respectively.The cardiac function was dynamic monitored by color Doppler ultrasound.The changes of morphology were observed after haematoxylin-eosin and Masson staining.CD31+ cell immunohistochemical staining was used to observe and calculate the microvascular density.The VEGF and b-FGF mRNA level were detected by realtime RT-PCR.RESULTS On 7,14 and 35 days,EF and FS in mSWT-H group were significantly higher than those in model group (P<0.01);LVEDV and LVESV were significantly lower than those in model group;EF and FS in mSWT-L group were significantly higher than those in model group on 14 and 35 days(P<0.01),LVEDV at 7,14 days and LVESV at 7,14,35 days were significantly lower than those in the model group (P<0.01);on the 7th day,the 4 indicators in the mSWT-H group were significantly improved compared with the mSWT-L group (P<0.01).Compared with the model group,mSWT-H and mSWT-L groups were significantly reduced myocardial fibrosis,myocardial histopathology was significantly improved,myocardial tissue CD31+ cell microvessel density was significantly increased (P<0.05),and the expression of VEGF and b-FGF mRNA were significantly up-regulated (P<0.05).CONCLUSION mSWT up-regulates the expression of VEGF and b-FGF in myocardial tissue and promotes angiogenesis,improves ventricular remodeling,thereby improving cardiac function in AMI rats.
