针刺联合中药熏蒸对踝关节骨折术后患者踝关节功能及骨代谢指标的影响

目的探讨针刺联合中药熏蒸对踝关节骨折术后患者踝关节功能及骨代谢指标的影响。方法以沈阳市第一人民医院收治的110例接受手术治疗的踝关节骨折患者作为研究对象,随机分为对照组和试验组,各55例。对照组给予常规康复训练,试验组在此基础上应用针刺联合中药熏蒸治疗,治疗3个月。比较两组总体疗效,观察两组踝关节量表(Olerud-Molander、Baird)评分变化情况、踝关节功能包括踝关节肌力和关节主动活动度以及骨代谢指标改善情况。结果试验组优良率高于对照组(70.91%vs.43.63%,P<0.05)。治疗后,两组患者Baird踝关节评分、Olerud-Molander功能评分均明显升高(P<0.05);踝关节肌力和关节主动活动度均提升(P<0.05);骨代谢指标血清骨钙素(BGP)、前胶原氨基端肽(PINP)、前胶原羧基端肽(PICP)、血清总骨Ⅰ型前胶原氨基端延长肽(Total-PINP)和Ⅰ型胶原羧基端肽β特殊序列(β-CTX)水平降低(P<0.05);试验组各指标改善均优于对照组(P<0.05)。结论针刺联合中药熏蒸具有活血止痛、化瘀消肿、补肾壮骨之效,作用于踝关节骨折术后患者,可有效恢复其踝关节功能,缓解关节疼痛、肿胀症状,调节骨代谢指标,对于踝关节骨折术后有较好的治疗效果。

Effect of acupuncture combined with Chinese medicine fumigation on ankle function and bone metabolism indexes of patients after ankle fracture operation

Objective To explore the effect of acupuncture combined with Chinese medicine fumigation on ankle function and bone metabolism indexes of patients after ankle fracture operation.Methods Totally 110 patients with ankle fractures undergoing surgical treatment in our hospital were selected as the subjects.All patients were randomly divided into two groups(control group and experiment group)with 55 cases in each group.Control group was treated with routine rehabilitation training,while treatment group was treated with routine rehabilitation training plus acupuncture combined with Chinese medicine fumigation.All patients were treated for 3 months.The overall curative effect of the two groups was compared,and the scores of Olerud-Molander and Baird,the ankle function(ankle muscle strength and active range of motion),and the levels of bone metabolism indexes of the two groups were observed.Results The excellence rate of experiment group was higher than that of control group(70.91%vs.43.63%,P<0.05).In both groups,the Baird score(ankle score)and Olerud-Molander score(function score)were significantly increased(P<0.05),the ankle muscle strength and active range of motion were improved(P<0.05),the levels of BGP,PINP,PICP,Total-PINP andβ-CTX were decreased significantly(P<0.05).The improvement in the indexes in experiment group was better than that in control group(P<0.05).Conclusion Acupuncture combined with Chinese medicine fumigation has the effects of activating blood circulation and relieving pain,dispersing stasis and reducing swelling,and tonifying kidney and strengthening bone.It can effectively restore ankle function,relieve joint pain and swelling,and adjust the level of bone metabolism indexes after ankle fracture operation.It has a good therapeutic effect on the postoperative state of ankle fracture.
