177例注射用头孢拉定致儿童血尿不良反应/事件文献分析

目的探讨头孢拉定特别是注射用头孢拉定致患儿血尿的临床特点及其原因,为临床合理用药提供参考。方法以＂头孢拉定＂、＂血尿＂、＂不良反应＂、＂儿童＂等为检索词,检索1994～2007年中国医院数字图书馆CHKD期刊全文数据库和中国期刊全文数据库,对收集到177例头孢拉定致儿童血尿不良反应进行统计与分析。结果检索到头孢拉定致儿童血尿177例,患儿均无血尿及肾脏病史,出现不良反应停用头孢拉定及对症处理后,血尿很快消失,故均诊断为＂药物性血尿＂。3岁以下患儿血尿发生率最高,静脉给药产生血尿的可能性更大,且用药浓度、剂量、滴注速度等因素也与血尿的产生有关。结论儿科临床用此药时应严格按适应证及说明书用法、用量用药,减少不良反应的发生。

Analysis of 177 Cases of Children's Hematuria Adverse Drug Reaction/Event Caused by Cefradine

Objective Reviewing and analyzing the clinical process of the children＇s hematuria caused by cefradine especially cefradine for injection, and also studying the etiology of this phenomena. Methods 177 patients＂ data were analyzed after using cefradine on the basis of related articles in China Hospital Knowledge Database owned by China Digital Library and Chinese Journals Full-text Database with the searching word such as cefradine, hematuria, adverse drug reaction（ADR） and children patients. Results Children patients with no kidney diseases were given symptomatic therapy after hematuria turned up, then completely recovered. The humaturia was diagnosed as ＂drug-related blood urine＂. Children not yet 3 ages were more likely to come to being hematuria ADR which was mainly associated with intravenous injection, overdose usage, over concentration, over speed of administer and so on. Conclusion To avoid the hematuria ADR, cefradine usage dose, concentration and speed of at-minister should be controlled strictly in children department.
