摘    要:摘要:目的 基于CMI指数调整DDD评价南京市第一医院科室抗菌药物使用水平。方法 收集2017—2019年南京市第一 医院出院病案首页数据,对科室CMI及DDD采用非整秩次方式编秩、计算秩和比、回归方程及分档排序。结果 对28个科室采 取CMI指数调整抗菌药物使用强度的秩和比法评价发现:①28个被评科室中仅有18个科室评价结果相同,占比64.29%;②CMI 调节DDD指标的综合评价比DDD单一评价下调级别的科室有5个;③CMI调节下DDD评价比单一评价上调级别的科室5个。结论 ①秩和比法可以通过区分高优和低优指标统一不同的指标量纲,形成综合评价;②CMI调节DDD指标的综合评价可以考虑到疾 病收治因素、搁置抗菌药物合理使用,为抗菌药物DDD的管控迅速提供管理线索;③通过CMI调节DDD指标的综合评价可以提 升管理效率,更加集中有效地对DDD进行管控。


RSR evaluation of CMI-index-adjusted antimicrobial use intensity in Nanjing First Hospital from 2017 to 2019
Abstract:Abstract Objective To evaluate the use level of antibacterial drugs in departments of Nanjing First Hospital based on CMI-adjusted antimicrobial drug use intensity(DDD). Methods The first page data of discharge medical records in Nanjing First Hospital from 2017 to 2019 were collected. The CMI and DDD were ranked by rank, rank sum ratio, regression equation, and classification. Results 28 departments were evaluated by the rank sum ratio (RSR) method with the CMI index to adjust the intensity of antimicrobial use. The results showed that: (1) Only 18 departments in 28 departments had the same evaluation results, accounting for 64.29%; (2)There were five departments which were adjusted by CMI and five departments adjusted by DDD were adjusted by CMI. Conclusion (1) The rank sum ratio method can unify the different index dimensions by distinguishing the high and low superiority indexes, and form a comprehensive evaluation; (2) the comprehensive evaluation of the CMI regulated DDD indexes can take into account the factors of disease admission and treatment, and put aside the rational use of antibiotics,and it can provide a prompt management clue for the management and control of antibiotics DDD. (3) through the comprehensive evaluation of CMI regulated DDD indexes, the management efficiency can be improved, and the DDD can be controlled more effectively.
