支气管肺泡灌洗治疗新生儿胎粪吸入综合症59例临床体会

目的探讨支气管肺泡灌洗治疗新生儿胎粪吸入综合征的临床疗效。方法将2008年10月至2011年2月收治的胎粪吸入综合征的新生儿59例随机分为治疗组25例和对照组24例,对照组患儿采用常规治疗方法,治疗组在对照组治疗的基础上给予支气管肺泡灌洗治疗。结果两组患儿经治疗后治疗组总有效率为88.0%,对照组总有效率为66.7%,两组治疗前后血气分析各项指标均有改善,其中pH值(pH)、氧分压(PaO2)、血氧饱和度(SaO2)下降,二氧化碳分压(PaCO2)升高,治疗组改善更为明显,两组治疗前后比较差异具有统计学意义(P<0.05)。说明治疗组效果优于对照组。结论支气管肺泡灌洗治疗新生儿胎粪吸入综合征疗效显著,操作简单,能争取到宝贵的关键时间,对MAS患儿的救治具有较高的临床实用价值。

Clinical experience of bronchoalveolar lavage in treatment of meconium aspiration syndrome of neonatal 59 cases

Objective To investigate the clinical efficacy of bronchoalveolar lavage of meconium aspiration syndrome.Methods Our hospital from October 2008 to February 2011 admitted to the neonatal meconium aspiration syndrome,59 cases were randomly divided into treatment group of 25 patients and a control group of 24 patients in the control group were treated with conventional therapy,the treatment group given on the basis of the control group bronchoalveolar lavage.Results The two groups of patients after treatment total effective rate was 88.0% in the control group,the total effective rate was 66.7%,blood gas analysis before and after treatment of the indicators have improved,which pH value(pH),partial pressure of oxygen(PaO2),oxygen saturation(SaO2) decreased carbon dioxide partial pressure(PaCO2) increased in the treatment group improved more significantly,the two groups before and after treatment was statistically significant difference(P<0.05).Shows effect of the treatment group was better than that of the control group.Conclusions Bronchoalveolar lavage meconium aspiration syndrome is significant,simple operation,the key to win valuable time,the treatment of MAS in children with high clinical value.
