难治性特发性癫痫的外科治疗

目的 总结对48例难治性特发性癫痫病人进行外科治疗的实践经验。方法 将特发性癫痫按临床表现结合神经电生理检查分为三种类型,即部分性发作、部分性发作继发全身性发作和全身性发作,并据此分类分别进行不同的手术方法。结果 本组病例外科治疗有效率为91.67％,不同手术方法有效率比较无显著性差异。结论 根据临床表现结合神经电生理检查并采用分类手术原则可能是治疗难治性特发性癫痫的有效途径。

Surgical Treatment of Intractable Eidiopathic Epilepsy

Objective To summarize surgical practice with 48 cases of intractable eidiopathic epilepsy (IEE) patients.Methods We demonstrated a clinical classification of IEE in this article,which were distinguished IEE into three types,namely partial seizures,partial secures secondarily becoming generalized and generalized seizures by means of clinical expression and neuro-electrophysiology detection.According to the different types,we selected three various operative method.Results It showed the effective rate of the surgical therapy is 91.67%,there were no significant differences in the effective rate among the different operations.Conclusions It is a possible effective way to adopt various operation for different IEE by clinical classification combining to clinical expression and neuro-eleetrpphysiology detection.
