| 摘 要： | 目的:探讨临床药师参与重症高龄患者综合治疗的药学监护工作要点。方法:临床药师参与1例重症高龄患者的治疗,对营养支持、镇痛治疗、鼻饲药物的给予以及抗感染治疗等多方面提出具体意见,为患者制订个体化的给药方案,对护理人员及患者家属进行具体的给药指导。结果:通过药学监护,临床药师为临床提供了合理用药方案,取得了良好的治疗效果。结论:临床药师与医师、护士共同组成治疗团队,有利于提高临床治疗水平,促进合理用药。

| Abstract: | OBJECTIVE：To approach the points of clinical pharmacists participating in pharmaceutical care for severe aged patients.METHODS：Clinical pharmacists participated in treatment for a severe aged patient and put forward suggestions on nutritional support,analgesia treatment,drugs by nasal feeding and anti-infection treatment,etc.Clinical pharmacists also developed individual dosage regimen and provided medication guidance for nurse and family members of patients.RESULTS：Through pharmaceutical care,the reasonable drug therapy was provided by the clinical pharmacist and the patient got satisfactory results.CONCLUSION：Clinical pharmacists,physicians and nurses form treatment group,which is benefit to improve clinical treatment and rational drug use. |
