| Abstract: | The authors elucidate the concept and significance of medication reeonciliation,indicate that pharmacists can play a vital role in improving health care and lowering costs through medication reconciliation, and advance the reform of clinical pharmacy education by medication reconciliation teaching. The aim of this paper is to train more clinical pharmacist through medication reconciliation teaching at each point of clinical care. |
