氯吡格雷联合阿司匹林治疗急性脑卒中的效果分析

目的探讨氯吡格雷联合阿司匹林治疗急性脑卒中的效果。方法以我院2012年1月至2013年7月收治的100例急性缺血性脑卒中患者为研究对象，随机分为观察组与对照组，两组患者均进行基础治疗，对照组使用阿司匹林，观察组使用氯吡格雷联合阿司匹林方案，比较两组患者临床治疗效果、体内炎症反应强度、神经功能缺损情况、血小板抑制率及不良反应发生率之间的差异。结果观察组总显效率78．00％，对照组总显效率56．00％，观察组总显效率显著高于对照组（x^2=5．47，P〈0.05）。治疗后，观察组hs—CRP、IL-6和PS水平分别为（2．68±1．69）mg/L、（8．47±2．39）μg/L和（9．01±1．06）ng/L，对照组分别为（4．52±1．85）mg／L、（10．01±3．32）μg/L和（11．01±1．23）ng／L，观察组hs—CRP、IL-6和PS水平显著低于对照组，差异有统计学意义（P〈0．05）。在治疗第3天和第14天，观察组患者神经功能缺损评分皆低于对照组，血小板抑制率均高于对照组，差异有统计学意义（P〈0．05）。观察组并发症发生率为16．00％，对照组为12．00％，差异无统计学意义（P〉0．05）。结论氯吡格雷联合阿司匹林可以有效治疗急性缺血性脑卒中，降低体内炎性反应强度，改善患者预后。

Combination of clopidogrel and aspirin in treatment of acute stroke

Objective To explore the efficacy of clopidogrel combined with aspirin in the treatment of acute stroke. Methods 100 patients with acute stroke were divided into an observation group and a control group. Conventional treatment plan was used in all the patients. The observtioan group were treated with clopidogrel and aspirin, and the control group with only aspirin. The differences in clinical efficacy, the intensity of inflammation, nerve function defect, platelet inhibition rate, and incidence of adverse reactions between the two groups were compared. Results The clinical efficacy of the observation group （78.00%） was better than that of the control group （56.00%）. After the treatment, the levels of hs-CRP, IL-6, and PS were （ 2.68 ± 1.69 ） mg/L, （ 8.47 ± 2.39 ） μg/L and （ 9.01± 1.06 ） ng/L in the observation group and were （ 4.52 ± 1.85 ） mg/L, （ 10.01± 3.32 ）μg/ L, and （ 11.01±1.23 ） ng/L in the control group, with statistical differences. Day 3 and 14 of the treatment, the nerve function defect was better and the platelet inhibition rate was higher in the observation group than in the control group. The incidence of complications was 16.00% in the observation group and 12.00% in the control group, with no statistical difference. Conelusions Clopidogrel combined with aspirin in the treatment of acute stroke is effective and can decrease inflammation intensity and improve the prognosis.
