法舒地尔与尼莫地平治疗蛛网膜下腔出血(SAH)后脑血管痉挛的效果对比

摘    要:目的 探讨法舒地尔与尼莫地平治疗蛛网膜下腔出血(SAH)后脑血管痉挛的临床效果.方法 选择本院2012年9月至2014年6月收治的92例SAH后脑血管痉挛患者为研究对象,随机分为对照组46例(尼莫地平治疗)和观察组46例(法舒地尔治疗).分析两组治疗后疗效及安全性.结果 用药后3周对脑血管痉挛观察,观察组脑血管痉挛发生率为4.35%,对照组为6.52%,差异无统计学意义(P>0.05);观察组治疗后Glasgow预后评分显示有效率为84.78%,对照组为69.57%,差异有统计学意义(P<0.05);观察组治疗后出现发热、头痛及面部充血等不良反应发生率为8.70%,对照组为23.91%,差异有统计学意义(P<0.05);结论 法舒地尔、尼莫地平对脑血管痉挛改善情况及预后均有较高疗效,但是法舒地尔的安全性较高,利于远期预后,值得临床推广.

Fasudil versus nimodipine in treatment of cerebral vasospasm after subarachnoid hemorrhage(SAH)
Abstract:Objective To explore the clinical effects of fasudil or nimodipine in the treatment of cerebral vasospasm after subarachnoid hemorrhage(SAH).Methods 92 patients with cerebral vasospasm after SAH treated at our hospital from September,2012 to June,2014 were selected as study objects and randomly divided into a control group (46 cases),treated with fasudil,and an observation group (46 cases),treated with nimodipine.The efficacy and safety were compared between these two groups after the treatment.Results After 3 weeks' treatment,the incidence of cerebral vasospasm was 4.35% in the observation group and 6.52% in the control group,with no statistical difference (P>0.05).After the treatment,Glasgow prognosis score showed that the effective rate was 84.78% in the observation group and 69.57% the control group,with a statistical difference (P<0.05).The incidence of adverse reactions,such as fever,headache,and facial congestion,was 8.70% in the observation group and 23.91% in the control group after the treatment,with a statistical difference (P<0.05).Conclusions Fasudil and nimodipine for the improvement and prognosis of cerebral vasospasm both has high clinical effect,but fasudil is safer and more beneficial to long-term prognosis,so it is worth being clinically generalized.
