46例新生儿HIE振幅整合脑电图与脐动脉血气、Apgar评分、头颅MRI相关性研究

目的 探讨振幅整合脑电图能否作为早期诊断新生儿缺氧缺血性脑病(HIE)的依据.方法 选择新生儿HIE患儿46例为观察对象,分析aEEG异常程度与脐动脉血气pH值和BE值的相关性,同时分析其与Apgar评分及头颅MRI的相关性.结果 ①aEEG用于判断新生儿HIE的敏感性为70.8％,特异性为96.9％,说明aEEG的异常程度与HIE的分度存在有显著的相关性;②aEEG异常程度与脐动脉血气比较差异有统计学意义(P＜0.05);③aEEG异常程度与Apgar评分比较差异有统计学意义(P＜0.05);④aEEG异常程度与生后3～7d内的头颅MRI检查异常程度紧密相关.结论 新生儿HIE振幅整合脑电图异常程度与脐动脉血气pH值,Apgar评分及头颅MRI密切相关,可作为作为早期诊断新生儿HIE的依据之一.

The correlation between amplitude integrated electroencephalogram and umbilical arterial blood gas,Apgar score,head MRI in 46 cases of neonatal HIE

Objective To investigate amplitude integrated electroencephalogram (aEEG) can be used as the basis for early diagnosis of neonatal hypoxic ischemic encephalopathy (HIE) or not.Methods 46 cases of neonatal HIE were selected as the observation object,analyzed the correlation between aEEG abnormal degree and pH and BE value of umbilical arterial blood gas,at the same time analyzed its correlation with Apgar score and cranial MRI.Results 1.The sensitivity of aEEG in the diagnosis of neonatal HIE was 70.8％,the specificity was 96.9％,which showed that there was significant correlation between aEEG abnormal degree and the degree of HIE;2.there was significant correlation between aEEG abnormal degree and umbilical arterial blood gas;3.there was significant correlation between aEEG abnormal degree and Apgar score;4.there was significant correlation between aEEG abnormal degree and the abnormal degree of head MRI within 3 to 7 days after the birth.Conclusion aEEG abnormal degree in neonatal HIE is closely related to pH value of umbilical artery blood gas,Apgar score,and cranial magnetic resonance imaging (MRI),can be used as one of the basis of early diagnosis of neonatal HIE.
