摘    要:目的 研究分析磁共振弥散成像在前列腺良恶性病变中的诊断价值.方法 对本院80例经病理证实为前列腺癌、前列腺增生的患者的DWI和ADC图像表现进行分析,测量病灶区ADC值,对两组ADC值进行比较分析.结果 46例前列腺增生主要发生在中央带腺体,在DWI上呈不均匀稍高信号,ADC值平均为(1.35±0.35)×10-3 mm2/s.34例前列腺癌中,16例病灶局限在外周带,8例发生在中央带,在DWI上呈明显高信号,平均ADC值为(0.99±0.25)×10-3 mm2/s.两组ADC值差异有统计学意义(t=-3.89,P< 0.05).结论 磁共振弥散加权成像,特别是ADC值定量分析对前列腺增生和前列腺癌的鉴别诊断有重要的价值.

Abstract:Objective To investigate the value of magnetic resonance diffusion weighted imaging in the differential diagnosis of benign and malignant prostate lesions.Methods The DWI and ADC images of 80 patients pathologically diagnosed with prostate cancer or benign prostatic hyperplasia were analyzed.The ADC value of the lesion area was measured,and the ADC values of the two groups were compared and analyzed.Results The 46 cases of benign prostatic hyperplasia occurred mainly in the central zones of the glands and had uneven slightly high signal in DWI and their mean value of ADC was (1.35±0.35)×10-3 mm2/s.Among the 34 cases of prostate cancer,16 cases were confined to peripheral zones;8 cases occurred in the central zones and had significantly high signal in DWI;the mean ADC value was (0.99-±0.25)× 103 mm2/s.There was a statistical difference in ADC value between these two groups (t=-3.89,P < 0.05).Conclusion Diffusion weighted imaging,especially ADC,has important value in the differential diagnosis of benign prostatic hyperplasia and prostate cancer.
