丹参川芎嗪注射液联合低分子肝素治疗急性脑梗死的疗效观察

摘    要:目的探讨丹参川芎嗪注射液联合低分子肝素钙注射液治疗急性脑梗死的临床疗效。方法选取唐山市第九医院2013年12月—2016年1月收治的80例急性脑梗死患者作为研究对象,按随机数字表法将所有患者分为对照组和治疗组,每组各40例。对照组皮下注射低分子肝素钙注射液5 000 U,2次/d。治疗组在对照组基础上静脉滴注丹参川芎嗪注射液,10 m L加入5%葡萄糖溶液250 m L,1次/d。两组患者均治疗28 d。观察两组患者的临床疗效,比较两组治疗前后血液流变学指标和美国国立卫生研究院卒中量表(NIHSS)评分。结果治疗后,对照组和治疗组的总有效率分别为75.00%、95.00%,两组比较差异具有统计学意义(P0.05)。治疗后,两组患者全血低切黏度、全血高切黏度、纤维蛋白、血浆黏度均明显降低,同组治疗前后差异均具有统计学意义(P0.05)。治疗后,治疗组这些观察指标的降低程度优于对照组,两组比较差异具有统计学意义(P0.05)。治疗7、14、28 d后,两组患者NIHSS评分均明显降低,同组治疗前后差异均具有统计学意义(P0.05)。治疗7、14、28 d后,治疗组NIHSS评分均低于对照组,两组比较差异具有统计学意义(P0.05)。结论丹参川芎嗪注射液联合低分子肝素钙注射液治疗急性脑梗死具有较好的临床疗效,可明显改善血液流变学指标,促进神经功能恢复,具有一定的临床推广应用价值。

Abstract:Objective To investigate the clinical efficacy of Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection combined with Low Molecular Weight Heparins Calcium Injection in treatment of acute cerebral infarction. Methods Patients (80 cases) with acute traumatic cerebral infarction in Tangshan Ninth Hospital from December 2013 to February 2016 were enrolled in this study, and were divided into control group and treatment group according to random number table, and each group had 40 cases. Patients in the control group were sc administered with Low Molecular Weight Heparins Calcium Injection 5 000 U, twice daily; Patients in the treatment group were iv administered with Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection on the basis of the control group, 10 mL added into 5% glucose solution 250 mL, once daily. The patients in two groups were treated for 28 d. After treatment, the clinical efficacies were evaluated, and hemodynamics indexes and NIHSS score in two groups before and after treatment were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 75.00% and 95.00%, respectively, and there was difference between two groups (P<0.05). After treatment, low blood viscosity, whole blood viscosity, plasma viscosity, and fibrinogen of two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). After treatment, the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). After treatment for 7, 14 and 28 d, NIHSS scores of two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). After treatment, NIHSS scores in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection combined with Low Molecular Weight Heparins Calcium Injection has a good clinical efficacy in treatment of acute cerebral infarction, can improve hemorheology indexes, and promote the recovery of neurological function, which has a certain clinical application value.
