原发性高血压脉压变化与窦性心律振荡及自主神经功能受损的作用机制研究

摘    要:目的分析原发性高血压脉压的变化对患者窦性心律振荡(HRT)及自主神经功能受损的影响。方法选择在本院确诊为原发性高血压的患者108例,按照其脉压的差值(以40mmHg为临界值)大小分为A、B两组,A组患者脉压值在40mmHg以上,共55例,B组患者脉压值不超过40mmHg,共53例。对两组患者的HRT的振荡初始(TO)及震荡斜率(鸭)进行检测并根据检测值比较两组患者的HRT情况。结果经检测,A组患者的HRT情况:TOI〉0及TS≤2.5ms/RR28例,TO〈0及TS〉2.5ms/RR27例;B组患者的HRT情况:TO≥0及TS≤2.5ms/RR48例,TO〈0及TS〉2.5ms/RR5例;当TS高于中性值时,表示窦性心律出现减速。经比较,A、B两组的HRT情况差异具有统计学意义(,=20.36,P〈0.01)。结论原发性高血压患者在脉压增大时能够引起窦性心律减弱或消失,从而导致HRT异常,并可以导致患者出现自主神经紊乱或受损情况。

Abstract:Objective To study the effect of primary hypertension pulse pressure change on HRT and autonomic nerve functional lesion. Methods 108 patients who were diagnosed as primary hypertension were di- vided into group A and B according to pulse pressure difference (critical value was 40 mmHg). 55 patients were in group A, the pulse pressure was higher than 40 mmHg, 53 patients were in group B, the pulse pressure was lower than 40 mmHg. TO and TS of HRT in two groups were tested and compared. Results In group A, 28 patients were TO~0 and TS~2. 5 ms/RR, 27 were TO 〈0 and TS 〉2. 5 ms/RR, in group B, 48 patients were TO~0 and TS~2. 5 ms/RR, 5 were TO 〈0 and TS 〉2. 5 ms/RR, when TS value was higher than neutral value, sinus rhythm slowdown appeared. HRT in group A and B were different significantly (X2 = 20. 36, P 〈 0. 01 ). Conclusion Sinus rhythm slowdown or disappearance appears when hypertension pulse pressure in- creases, leading to abnormal HRT and autonomic nerve functional lesion.
