血清胱抑素C测定在儿童肾病综合征中的应用

摘    要:目的探讨血清胱抑素C(Cys—c)在儿童肾病综合征中的应用价值,并分析经糖皮质激素治疗后血清Cys-C浓度的变化。方法对40例儿童肾病综合征患儿经糖皮质激素治疗前后的血清Cys-C、血清肌酐(SCr)和尿素(Urea)含量进行分析检测,并与健康对照组比较。结果儿童肾病综合征组的血清Cys-C、Urea和SCr含量均显著高于正常对照组(尸〈0.01)。儿童肾病综合征组经激素治疗前Cys-C、Urea和SCr的阳性率分别为85.0%、52.5%和35.0%,治疗后分别为27.5%、20.0%和17.5%。儿童肾病综合征激素治疗前Cys-C较Scr和Urea浓度已显著增高(P〈0.01),儿童肾病综合征激素治疗后有20%患者的Cys-C浓度较激素治疗前增高。结论测定血清Cys-C对诊断早期儿童肾病综合征病比Scr和Urea更具临床意义,使用大量的糖皮质激素治疗会影响Cys-C的浓度测定。

Abstract:Objective To investigate the application of serum cystatin C (Cys-C) assay in children with nephrotic syndrome, and detect the changes in Cys-C levels after glucocorticoid therapy. Methods The levels of Cys-C, serum creatinine (SCr) and Urea in 40 children with nephrotic syndrome (the study group) were detected before and after glucocorticoid treatment, then the data was compared with that of 38 healthy individuals (the control group). Results The levels of serum Cys-C, Urea and SCr in the study group were significantly higher than that in the control group (P<0.01). The positive rates of Cys-C, Urea and SCr in the study group were 85.0%, 52.5% and 35% before treatment, and 27.5%, 20.0% and 17.5% after treatment. In the study group, the levels of Cys-C before treatment were significantly increased compared with that of Scr and Urea (P<0.01), and 20% of the patients has significantly higher Cys-C levels after treatment. Conclusion Cys-C assay has greater value in the diagnosis of nephrotic syndrome in children than Urea and Scr. Utility of high-dose of glucocorticoid may influence the levels of cystatin C.
