经椎旁入路椎间盘镜治疗极外侧型腰椎间盘突出症五例

目的探讨经椎旁入路椎间盘镜治疗极外侧型腰椎间盘突出症的临床疗效。方法 5例患者采取俯卧位,C形臂定位后经椎旁入路运用椎间盘镜技术于神经根管外口找到神经根,显露突出椎间盘并予以摘除。结果平均手术时间90min,平均术中出血55ml,平均住院时间14d,全组手术切口均一期愈合。随访时间6～24个月。疗效按Nakai分级评定,优3例,良2例。结论该术式创伤小,不累及椎管,脊柱稳定性破坏小,治疗椎间孔外型腰椎间盘突出症疗效明确。

Clinical effect of microendoscopic discectomy through the paravertebral processes in the treatment of 5 cases of far lateral disc herniation

Objective To investigate the clinical effect of microendoscopic discectomy in the treatment of far lateral disc herniation through the paravertebral processes.Methods With the patients in a prone position,the C-arm was positioned and the microendoscopic discectomy was used through the paravertebral processes to locate the nerve root from the nerve root canal.The intervertebral disc was resected as it exposed.Results The average operation time was 90 minutes,the average bleeding volume during operation was 55 ml and the average length of hospital stay was 14 days.All the incisions obtained primary healing.The five patients were followed up for 6~24 months.According to Nakai standard,3 cases were scored excellent,and 2 cases were scored good.Conclusion Microendoscopic discectomy through the paravertebral processes is effective for treating far lateral disc herniation,which causes no harm to the spinal canal and the spinal stability,with minimal injury.
