EGR-1及MMP-9在子宫内膜异位症中的表达及意义

摘    要:目的 研究早期生长反应基因-1(EGR-1)与金属基质蛋白酶-9(MMP-9)在子宫内膜异位症患者异位内膜与在位内膜细胞中的表达情况及临床意义.方法 收集45例2014年5月至2015年3月于四川省自贡市妇幼保健院确诊的子宫内膜异位症患者的异位内膜及在位内膜,同期收集30例非子宫内膜异位症患者的正常内膜组织.通过免疫组化二步法对三种内膜组织中EGR-1、MMP-9的表达情况进行分析.结果 EGR-1在子宫内膜异位症患者异位内膜(84.44%)与在位内膜(73.33%)腺上皮细胞中的阳性检测率显著高于正常内膜组(40.0%),差异有统计学意义(P<0.05),但两者差异无统计学意义(P>0.05);而不同内膜间质细胞中EGR-1的阳性检出率异位内膜为48.89%,在位内膜为44.44%,正常内膜为43.33%,三者比较差异无统计学意义(P>0.05);异位内膜和在位内膜腺上皮细胞(在位内膜88.89%、异位内膜64.44%)及间质细胞(在位内膜66.67%、异位内膜42.22%)中MMP-9的阳性检出率均显著高于正常内膜组(腺上皮细胞33.33%、间质细胞16.67%),差异均有显著统计学意义(P<0.01),且异位内膜组显著高于在位内膜组,差异有统计学意义(P<0.05).结论 检测EGR-1、MMP-9的表达可用于判断子宫内膜异位症的侵袭转移和评估预后.

Abstract:Objective To investigate the expression of early growth response gene-1 (EGR-1) and matrix metallo proteinase-9 (MMP-9) protein in the ectopic and eutopic endometrium in endometriosis (EMs) patients and its clinical significance. Methods The expression of EGR-1 and MMP-9 protein in ectopic and entopic endometrium tissues from 45 patients with EMs and endometrium tissues from 30 patients with normal endometrium in Zigong Municipal Maternal and Child Health Hospital from May 2014 to March 2015 were detected by immunohistochemis-try. Results The positive rate of expression of EGR-1 in glandular cell in ectopic (84.44%) or eutopic endometri-um (73.33%) of the patients with EMs were significantly higher than that in control group (40.0%) (P<0.05), but no signifi-cant difference between ectopic and eutopic endometrium was found (P>0.05). There were no significant differences in the positive expression rate of EGR-1 in stromal cells in ectopic endometrium (48.89%), eutopic endometrium (44.44%) or normal endometrium (43.33%) (P>0.05);The positive expression rate of MMP-9 in glandular and stromal cells in ectopic (glandular cells 88.89%, stromal cells 66.67%) or eutopic endometrium (glandular cells 64.44%, stromal cells 42.22%) of the patients with EMs were significantly higher than those in control group (glandular cells 33.33%, stromal cells 16.67%) (P<0.01), and the positive expression rate in ectopic endometrium were significantly higher than in that in eutopic endome-trium(P<0.05). Conclusion The expression of EGR-1 and MMP-9 in ectopic and eutopic endometrium may play an im-portant role in the invasion of endometriosis and metastasis of EMs as well as evaluation of prognosis .
