心脏超声筛查在胎儿先天性心脏病中的应用价值

摘    要:目的 探讨心脏超声筛查在胎儿先天性心脏病中的临床应用价值.方法 回顾性分析2013年1月至2015年1月期间来哈励逊国际和平医院进行胎儿超声筛查的13589例受检对象的临床资料,所有孕妇均接受产前超声检查和胎儿心脏专项检查,分析超声检查的结果 .结果通过产前心脏超声检查发现,13589例胎儿中有180例存在先天性心脏异常,检出率为1.3%,其中室间隔缺损42例,占23.3%;法洛四联征34例,占18.9%;心内膜垫缺损30例,占16.7%;三尖瓣下移25例,占13.9%;大动脉转位19例,占10.6%;肺动脉瓣狭窄伴心室发育不全15例,占8.3%;右室双出口10例,占5.6%;主动脉弓狭窄5例,占2.8%.产后检查172例符合产前诊断,其中漏诊5例,误诊3例,诊断符合率为95.6%,漏诊率为2.8%,诊断偏差率为4.4%.产前超声检查结果与产后新生儿检查结果或尸检结果比较差异无统计学意义(P>0.05).结论 胎儿心脏超声筛查对先天性心脏病具有较高的诊断价值.

Abstract:Objective To investigate the clinical value of echocardiography in congenital heart disease. Methods The clinical data of 13589 patients undergoing ultrasound screening from January 2013 to January 2015 in our hospital were analyzed retrospectively, the pregnant women were received prenatal ultrasound and cardiac spe-cial inspection, and results of ultrasound screening were analyzed. Results A total of 180 cases of fetal congenital cardiac abnormalities were found in 13589 cases by fetal heart ultrasound screening, with the detection rate of 1.3%, including 42 (23.3%) of ventricular septal defect, 34 (18.9%) of tetralogy of fallot, 30 (16.7%) of the endocardial cushion defect, 25 (13.9%) of tricuspid valve down, 19 (10.6%) of arteries transposition, 15 (8.3%) of pulmonary ste-nosis associated with ventricular hypoplasia, 10 (5.6%) of double outlet of right ventricular and 5 (2.8%) of aortic arch stenosis. The results of 172 cases of postpartum examination accorded with prenatal diagnosis, among on them, 5 cases of missed diagnosis, 3 cases of misdiagnosis. The diagnosis coincidence rate was 95.6%, the missed diagnosis rate was 2.8%, diagnostic rate of deviation was 4.4%. There was no significantly difference between prenatal ultraso-nography and postpartum ultrasonography or autopsy (P>0.05). Conclusion Fetal heart ultrasound screening for congenital heart disease has a higher diagnostic value, and it is worthy of clinic application.
