基于医护一体化有效沟通的移动视频探视在PICU中的应用

目的：探讨在儿童重症监护病房(PICU)利用移动视频探视系统结合医护一体化的有效沟通实施的方法和效果。方法选择2014年8月至2015年1月在PICU住院的120名患儿家属为对照组，2015年2月至2015年7月在PICU住院的131名患儿家属为观察组。对照组采用单一的视频探视方法，观察组在视频探视的基础上，评估患儿家属的需求，组成医护协同小组，完善“沟通—探视—沟通”工作制度，制定探视指引，确认探视流程，实施医护一体化的有效沟通，反馈探视沟通效果。采用焦虑自评量表和EMPATIC-30PICU患儿家属满意度测评量表对两组患儿家属进行问卷调查。结果对照组均发放120份问卷，收回115份，有效回收率为95.8%；观察组均发放131份问卷，收回128份，有效回收率为97.7%。观察组患儿家属焦虑程度为(49.4±5.9)分，明显低于对照组的(57.3±6.8)分，差异有统计学意义(P<0.05)。满意度调查结果显示，观察组家属对治疗照护、专业态度、父母参与、组织和信息5个维度的满意度分别为(4.9±0.7)分、(4.7±0.6)分、(5.0±1.2)分、(4.3±0.9)分和(5.0±1.1)分，均高于对照组的(3.2±0.9)分、(3.4±1.0)分、(3.1±0.8)分、(3.0±0.7)分和(3.2±0.8)分，差异均有统计学意义(P<0.05)。结论在PICU进行移动视频探视，结合医护一体化有效的沟通，动态了解患儿家属的心理需求，提供心理支持，缓解了家属的焦虑情绪，体现人文关怀，提高患儿家属的满意度，改善就医感受，有利于构建和谐的护患关系。

Application of mobile video visiting system based on effective communication of nurse-physician collaboration in

Objective To explore the application of mobile video visiting system based on effective communi-cation of nurse-physician collaboration in pediatric intensive care unit (PICU). Methods A total of 120 family mem-bers of hospitalized children in PICU from August 2014 to January 2015 were selected as the control group, and 131 fam-ily members of hospitalized children in PICU from February 2015 to July 2015 were chosen as the observation group. A single video visiting method was used in the control group. However, the comprehensive video visiting method was car-ried out based on evaluating the needs of children's family members, forming the nurse-physician collaboration, perfect-ing the system of“communication-visitation-communication”, customizing the visiting guidelines, confirming the vis-iting pipe, implementing the effective communication between nurse and physician, and feedbacking visiting communi-cation effect. Questionnaire survey was conducted on the families’members of the two groups by using the Self-Rating Anxiety Scale (SAS) and EMPATIC-30. Results In the control group, 120 questionnaires were sent out and 115 ques-tionnaires were retrieved, with the effective recovery rate of 95.8%. In the observation group, 131 questionnaires weredispatched and 128 questionnaires were recovered, with the effective recovery rate of 97.7%. The degree of anxiety of the family members in the observation group was significantly lower than that of the family members in the control group (P<0.05). Satisfaction survey results showed that the satisfaction rating scale of treatment care, professional atti-tude, parental involvement, organization and information in the family members in the observation group were respec-tively (4.9±0.7), (4.7±0.6), (5.0±1.2), (4.3±0.9), (5.0±1.1), which were significantly higher than those in the family mem-bers in the control group of (3.2±0.9), (3.4±1.0), (3.1±0.8), (3.0±0.7), (3.2±0.8), and the differences were statistically sig-nificant (P<0.05). Conclusion In PICU, mobile video visitation combined with the integration of effective communica-tion of nurse-physician collaboration, can dynamically understand the psychological needs of the children's families, pro-vide psychological support, relieve the anxiety of family members, reflect the humanistic care, improve the satisfaction of the children's families and the medical experience, which is conducive to building the harmonious relationship be-tween nurses and patients.
