子宫内膜异位症在位、异位内膜COX-2表达及腹腔液PGE2含量的研究

目的 通过测定子宫内膜异位症患者腹腔液中的前列腺素E2（PGE2）水平，在位、异位内膜组织环氧合酶-2（COX-2）的表达情况，以及它们二者之间的相关性，探讨COX-2、PGE2与子宫内膜异位症发病的相关性。。方法 采用放射免疫法，测定28例子宫内膜异位症患者（研究组）及22例浆膜下子宫肌瘤或单纯卵巢囊肿患者（对照组）腹腔液PGE2水平；采用免疫组化SP法测定研究组在位、异位内膜，以及对照组子宫内膜COX-2表达。结果 （1）研究组Ⅰ-Ⅱ期、Ⅲ-Ⅳ期腹腔液中PGE2水平均高于对照组（P〈0.05），而Ⅰ-Ⅱ期、Ⅲ-Ⅳ期相比较差异无统计学意义（P〉0.05）。（2）两组腹腔液PGE2水平均表现为在月经周期的增生期、分泌期，差异无统计学意义（P〉0.05）；研究组PGE2水平无论增生期、分泌期均高于同期的对照组（P〈0.05）。（3）两组在位内膜COX-2表达均为分泌期高于增生期（P〈0.05）；异位内膜COX-2表达增生期、分泌期差异均无统计学意义（P〉0.05）；研究组在位、异位内膜COX-2表达无论增生期、分泌期均高于同期的对照组内膜（P〈0.05）。（4）研究组在位、异位内膜的COX-2表达均与AFS-r分期无相关性（P〉0.05）。（5）腹腔液中PGE2浓度与异位内膜组织中COX-2表达无统计学相关性（r=0.30,P〉0.05）。结论 COX-2在在位、异位子宫内膜的过表达以及腹腔液中PGE2水平的异常升高可能是子宫内膜异位症发生、发展的原因之一。
