脂联素水平与胎儿生长发育之间的相关性探讨

摘    要:目的探讨脂联素在IDJL生长发育中的作用。方法选择2008年2月至2009年2月出生的足月剖宫产儿105例,根据出生体质量不同分为小于胎龄儿(SGA)组20例、适于胎龄儿(AGA)组55例和大于胎龄儿(LGA)组30例。采用放射免疫分析法检测脐血和母血脂联素水平;应用免疫比浊法检测脐血三酯甘油(TG)、总固醇(TCH)、低密度脂蛋白-胆固醇(LDL—C)、高密度脂蛋白-胆固醇(HDL—C)的水平;测量新生儿生长参数(身长、体质量、胎盘重量);计算体质量指数(BMI);并分析脐血脂联素水平与母血脂联素、新生儿生长参数,脐血血脂指标之间的相关性。结果(1)不同性别新生儿脐血脂联素之间无明显差异(P〉0.05).(2)3组新生儿脐血脂联素水平之间具有明显差异(P〈0.01),由高到低依次为LGA〉AGA〉SGA,而母血脂联素之间无明显差异(P〉0.05).(3)SGA组、LGA组新生儿脐血脂联素与新生儿体质量、BMI、胎盘重量均呈明显正相关(P〈0.05),但与脐血TG、TCH、LDL—C、HDL—C等各项血脂指标无关(P〉0.05).结论脂联素在胎儿宫内生长发育过程中起重要调节作用,可作为评价胎儿生长发育及营养状态的临床指标之一,同时为了解出生后的生长发育趋势及预防某些与肥胖相关的代谢性疾病的的发生提供新的途径。

Abstract:Objective To investigate the umbilical adiponectin levels with fetus growth and development. Methods One hundred and five full-term newborns by cesarean section from Feb. 2008 to Feb. 2009 were divided into 3 groups: the small for gestational age (SGA) group (n=20), the appropriate for gestational age (AGA) group (n=55) and the large for gestational age (LGA) group (n=30). Blood samples were collected from the vein of puerperal women and the umbilical cord of newborns; the blood adiponectin levels were examined by radio immunoassay and cord blood lipids (TG, TCH, LDL-C and HDL-C) were assayed with turbidlmetry. Neonatal growth parameter (body height, body weight, placental weight) were measured and body mass index (BMI) was calculated. The correlation of umbilical adiponectin levels with puerperal blood adiponectin levels, neonatal growth parameter, cord blood lipids was analyzed. Results There was no significant difference in umbilical adiponectin levels between sexes of newborns (P〉0.05). The umbilical adiponectin level in SGA group was highest and that in LGA group was lower, there were significant differences among three groups (P〈0.001); however, there was no difference in puerpera blood adiponectin levels among three groups (P〉0.05). The umbilical adiponectin levels in SGA and LGA groups were positively correlated with the body weight, BMI and placental weight, but not correlated with cord blood lipids levels (P〉0.05). Conclusion Cord blood adiponectin levels are correlated with fetus growth and development, which suggests that they can be used as clinical index to evaluate fetus growth, development and nutritional status.
